Retatrutide is a synthetic peptide of 39 amino acids, with a 20-carbon fatty diacid attached to lysine 17 through a short linker.1 Eli Lilly and Company discovered and developed it under the code LY3437943.23 In laboratory cell tests, the single molecule activated three receptors: GIP, GLP-1 and glucagon.2 Its activity was balanced at the glucagon and GLP-1 receptors and higher at the GIP receptor.2

Research highlights

  • Mouse studies: in obese mice, retatrutide lowered body weight and improved blood sugar control.2 Glucagon receptor activity added a rise in energy expenditure.2
  • Phase 1b, 72 adults with type 2 diabetes: the half-life was about 6 days.9 Over 12 weeks, HbA1c fell up to 1.6 percentage points more than with placebo.9
  • Phase 2, 338 adults with obesity or overweight: average weight fell 8.7% to 24.2% by dose over 48 weeks, versus 2.1% on placebo.10
  • Phase 2a substudy, 98 adults with fatty liver disease: liver fat fell 82.4% at the top dose over 24 weeks, versus a 0.3% rise on placebo.11
  • Phase 3 TRIUMPH-1, 2,339 adults with obesity and without diabetes: average weight fell 17.6% to 25.0% by dose over 80 weeks, versus 3.9% on placebo.12
  • Phase 3 TRANSCEND-T2D-1, 537 adults with type 2 diabetes: HbA1c fell 1.69 to 1.94 percentage points over 40 weeks, versus 0.81 on placebo.13

Regulatory status

As of October 2026, no regulator has approved retatrutide, and Lilly calls it a phase 3 investigational molecule.3 FDA states that retatrutide cannot be used in compounding under federal law, is not a component of any FDA-approved drug and has not been found safe and effective for any condition.15

FDA has sent warning letters to online sellers of research-labeled retatrutide, including letters in December 2024 and March 2026.161718 In each letter, FDA said evidence from the seller's own website showed the products were meant as drugs for people, despite research-use labels, which made them unapproved new drugs.161718 FDA says such products have been sold directly to consumers with dosing instructions, and it urges consumers not to buy them because they are of unknown quality and may be harmful.15

Lilly plans to submit retatrutide for US review in the first quarter of 2027, and says neither approval nor that timeline is guaranteed.193 In sport, WADA's 2026 Prohibited List does not name retatrutide.20 Its class S0 bans at all times any substance not covered elsewhere on the List that has no current approval for human therapeutic use, such as drugs in clinical development.20 D2C Peptides sells this vial for laboratory research only.
