Research goal
Sermorelin and muscle growth
No study reviewed here shows sermorelin building muscle mass. In 11 men aged 64 to 76, six weeks of nightly injections improved 2 of 6 strength measures, but DEXA muscle mass did not change.1 The evidence comes from small, short studies in older adults, and a 2026 review judges benefits in healthy people uncertain.2

What sources report
Sermorelin binds the GHRH receptor and stimulates the release of growth hormone (GH).10 Whether that adds muscle has been measured in only two small studies, both in older adults, and one of them used a related analog rather than sermorelin. Below is the research on sermorelin for muscle growth, study by study, with each evidence level. For the compound itself, see What is Sermorelin?; for other compounds on this goal, see Peptides and muscle growth.
Strength and muscle mass in older men
Evidence level: before-and-after study (n=11). The participants were 11 men aged 64 to 76, healthy, not obese and with low IGF-1, who self-injected each night at home for 6 weeks.1 Of six strength measures, two improved (upright row and shoulder press), and so did one endurance test, the abdominal crunch.1 Muscle and fat by DEXA, and muscle histology (tissue examined under a microscope), were unchanged.1 GH at night went up; IGF-1 did not.1
The abstract describes no control group, so the design cannot show that the treatment caused these changes. The gains reached P < .02 (upright row), P < .04 (shoulder press) and P < .03 (crunch), and the remaining four strength measures showed no significant change.1
The schedule question
This muscle study gave one injection a night, a schedule its authors suggested is less effective than several doses a day.1 The schedule that did lift IGF-1 in older men, the higher of two doses twice a day for 14 days, was tested only for hormone and safety measures, not muscle.11 No study here, then, measured muscle on the schedule that moved IGF-1. Study doses are listed on Sermorelin dosage.
Lean mass with a related analog
Evidence level: single-blind trial, 4 weeks of placebo first (n=19). The analog, [Nle27]GHRH(1-29)-NH2, is related to sermorelin but is a different compound. Ten women and 9 men aged 55 to 71 injected it each night for 16 weeks.3 Within 2 weeks their IGF-1 had risen.3 Men gained lean body mass and women did not, and no other body-composition measure moved.3 Well-being and libido improved in the men only.3 The sources here report no strength outcome from this trial.
Women
Women enter the muscle evidence only through that analog trial. The 10 women had thicker skin, but their lean mass did not change.3 Insulin sensitivity, well-being and libido did not change significantly in women.3 No sermorelin study in these sources measured muscle in women. More: Peptides and women's health.
GH and IGF-1 levels in older men
Evidence level: randomized dose-order trial (n=19, 9 of them young controls). Ten nonobese older men took each of two doses twice a day for 14 days.11 On the higher dose, their 24-hour GH, GH peaks and IGF-1 reached young-adult levels.11 What the sources capture from this trial is hormones and safety tests, not muscle. The older men also started with lower testosterone than the young men.11
Combinations with GH-releasing peptides
Evidence level: narrative review and retrospective chart review (n=14). GH-releasing peptides such as GHRP-2 and GHRP-6 use a receptor other than the GHRH receptor, and giving GHRH with a GHRP released GH synergistically.7 The chart review started from the records of 105 men on testosterone therapy who were seeking lean mass and fat loss, all prescribed sermorelin, GHRP-2 and GHRP-6 together.8 For the 14 men who qualified, average IGF-1 went from 159.5 to 239.0 ng/mL across a mean of 134 days.8 The paper reports no muscle result: body measurements were not available for most men at follow-up.6 No safety data for these combinations appear in the sources reviewed here, and FDA places GHRP-2, GHRP-6 and ipamorelin in its significant-safety-risk category for compounding.9 WADA classes CJC-1295 as a GHRH analogue and Ipamorelin as a growth hormone secretagogue,4 and a blend of the two is profiled at What is CJC-1295 + Ipamorelin?
How this differs from growth hormone itself
Somatropin (prescription HGH) is a growth hormone analog that replaces the hormone, and it treats growth failure from GH deficiency and some other conditions.12 Sermorelin was approved only for GH-deficient children and as a diagnostic, and both approvals ended in 2009.13 In these sources, head-to-head data exist only for children's height, not muscle: in a randomized trial of 60 GH-deficient children, those on GH grew 14.6 cm a year against 9.2 and 9.3 cm on the two GHRH(1-29) doses.14 Children's growth says nothing direct about adult muscle. Background: What is HGH?
Sport and performance
Athletes face a blanket ban: WADA prohibits sermorelin both in and out of competition.4 Reviewing GH-axis peptides used for performance, a 2026 paper found fitness and bodybuilding websites listing regimens that differ widely from those in studies.2 The same paper flags uncertainty over what unregulated products contain, in what amount, and what they are stacked with.15 A 2026 Sports Medicine review adds that many unapproved peptides look favorable in animal models while rigorous human safety data stay scarce.5
Safety signals that matter for training
For GH-axis peptides as a class, a 2026 review reports muscle and joint aches, fluid retention, blood sugar disturbance and injection-site reactions.15 For sermorelin in particular, a 1999 drug review reported short-lived facial flushing and injection-site pain as the most common adverse events.16 Long-term safety data do not exist.2 Full list: Sermorelin side effects.
What is and is not known
Known, from small studies in older adults:
- Sermorelin raises GH, and the higher of two tested doses raised IGF-1 in older men.11
- One 6-week before-and-after study saw 2 of 6 strength measures improve with no change in DEXA muscle mass.1
- A related analog added lean mass in men only, over 16 weeks.3
- Fasting glucose, urinary C-peptide, blood pressure and routine blood tests did not change over 14 days at each dose, although phosphate rose.11
Not known:
- Whether repeated use blunts the effect. In a 6-month trial, 39 of 40 children formed GHRH antibodies, though antibody levels did not track with growth.14
- What happens after stopping. These sources hold no adult data on stopping; in children, the antibodies had nearly vanished 9 months after treatment ended.14
- Whether sermorelin builds muscle in healthy adults. No controlled trial here shows it, and a 2026 review judges benefits in healthy people uncertain.2
- Its effect in younger adults or athletes. Every muscle result comes from people aged 55 to 76.13
- Long-term safety: a 2026 review notes that no long-term safety data are available.2
No sermorelin approval ever covered performance or body composition, and a 2026 review confirms none exists today.132 Compounded drugs carry no FDA approval, and FDA has treated research-labeled sermorelin as an unapproved drug when the seller's website showed it was intended for people.1718

Frequently asked questions
Does sermorelin build muscle?
Not on the evidence so far. Eleven men aged 64 to 76 given nightly injections for 6 weeks did better on 2 of 6 strength tests and one endurance test, while DEXA muscle mass and muscle tissue under the microscope did not change.1 Only a related analog, not sermorelin, added lean mass, and only in the 9 men, over 16 weeks.3 A 2026 review finds clinically meaningful benefits in healthy people uncertain.2
Is sermorelin banned in sports?
Yes. Under S2.2.4 of WADA's 2026 Prohibited List, growth hormone releasing factors, including sermorelin, CJC-1295 and tesamorelin, are banned at all times, whether in or out of competition.4 Separately, a 2026 Sports Medicine review warns that rigorous human safety data for unapproved peptides are scarce, with potential for serious harm.5
Does sermorelin mess with your testosterone?
No sermorelin-only testosterone data appear in the sources reviewed here. The closest evidence is a chart review of 14 men who were already on testosterone therapy and added sermorelin, GHRP-2 and GHRP-6: their LH, FSH and estradiol did not change significantly.6 Their testosterone climbed from day 90, but since all of them were taking testosterone, the rise cannot be pinned on the peptides.6 More: Peptides and testosterone.
How does sermorelin differ from ipamorelin and other GH-releasing peptides?
They act on different receptors. GH-releasing peptides such as GHRP-2 and GHRP-6 do not bind the GHRH receptor that sermorelin uses, and in research, GHRH and a GHRP given together released GH synergistically.7 WADA lists ipamorelin among growth hormone secretagogues, separate from GHRH analogues such as sermorelin.4 The one combination study here, a chart review, measured hormones only and cannot isolate sermorelin's part.8 FDA places GHRP-2, GHRP-6 and ipamorelin in its significant-safety-risk category for compounding.9
