Benefits and uses

Sermorelin benefits and uses

The documented benefits of sermorelin are narrow: in GH-deficient children it sped up growth, and a single IV dose served as a test for GH deficiency.12 In 10 older men, the higher of two doses raised GH and IGF-1 over 14 days.3 A 2026 review calls the evidence for benefits in healthy people limited and largely indirect.4

What sources report

Each finding below names its evidence level. The sources are human studies published from 1992 to 1997, a 1999 drug review, a 2006 editorial, a 2017 chart review and a 2026 review. The compound is introduced in What is Sermorelin?

Growth in GH-deficient children

Evidence level: human trials, including a multicenter open-label trial (n=110) and two randomized trials (n=60 and n=43). This is the use FDA approved in 1997, for idiopathic GH deficiency in children with growth failure.89

  • Across 110 previously untreated prepubertal children (86 analyzed for efficacy), average height velocity went from 4.1 cm per year before treatment to 8.0 at 6 months and 7.2 at 12 months.1
  • At 6 months, 74% of those children were rated good responders.1
  • Growth hormone did better head to head: in the randomized trial of 60 children, the GH group grew significantly faster than either GHRH group.7
  • In the 43-child trial, the higher dose split into three daily injections gave growth comparable to GH, while the lower dose gave the least growth.10
  • A nasal spray form doubled lower-leg growth rate at 6 weeks in a pilot of 8 children, but the effect then faded and 6-month height velocity did not rise.11
  • A 1999 review reported smaller height gains with sermorelin given by infusion or in 3 daily doses than with once-daily somatropin; the approved once-daily regimen was never compared directly. It said the effect on final adult height had not yet been determined.2

Testing for GH deficiency

Evidence level: narrative review of human studies; former FDA-approved diagnostic (1990). FDA approved the 0.05 mg Geref product in 1990 for evaluating the pituitary's ability to secrete growth hormone.129 The 1999 review called the single IV test rapid and relatively specific when used with conventional tests.2 It also noted that a normal response cannot rule out GH deficiency caused by a problem in the hypothalamus.2

Short-term GH release

Evidence level: human pharmacology study (n=30). In 30 healthy men aged 19 to 43, IV doses of 1 to 2 mcg/kg gave maximal GH release, and GH stayed raised for about 3 hours although the peptide itself cleared quickly.13 Drug database entries describe Geref's GH-releasing effect in humans as similar to that of the full 44-residue hormone.14

GH and IGF-1 in older men

Evidence level: two small human studies (10 treated older men with 9 young controls; 11 older men).

  • In 10 nonobese older men compared with 9 young men, the higher of two doses raised mean 24-hour GH and IGF-1 until the age groups no longer differed.3
  • In 11 men aged 64 to 76, nightly injections for 6 weeks raised nighttime GH, but IGF-1 did not change.5

Strength and body composition

Evidence level: one before-and-after study (n=11); one related-analog trial (n=19).

  • In the 6-week study, upright row and shoulder press (2 of 6 strength measures) and an abdominal crunch endurance test improved.5 Weight, BMI, waist-to-hip ratio, DEXA muscle and fat, and muscle histology did not change.5
  • In the related-analog trial, lean body mass rose in men only, with no other changes in body composition.6 Insulin sensitivity improved in men but not in women.6

IGF-1 with a three-peptide combination

Evidence level: retrospective chart review (n=14). In men already on testosterone therapy who took sermorelin with GHRP-2 and GHRP-6, mean IGF-1 rose from 159.5 to 239.0 ng/mL over a mean of 134 days.15 Men also taking drugs that lower estrogen or block its receptor had smaller rises.15 Sermorelin was never given alone, so its own share of the effect is unknown.1516

Well-being and libido

Evidence level: one related-analog trial (n=19). General well-being and libido improved in men but not in women.6 No sermorelin trial in the sources reviewed here measured either outcome.

Uses, past and present

  • Approved uses (ended): a diagnostic from 1990 and a treatment for GH-deficient children from 1997, both withdrawn effective June 18, 2009.1289
  • Compounded prescriptions: a 2022 recall notice listed sermorelin among one pharmacy's compounded products and said that group is typically prescribed for uses that include age management.17 Compounded drugs are not FDA-approved.18
  • Off-label prescribing: a 2006 editorial on sermorelin for adult-onset GH insufficiency, written while Geref was still approved, noted that off-label prescribing of sermorelin was not prohibited by federal law.19
  • Research use: the vial on this site carries a research-use-only label. FDA has treated research-labeled sermorelin as an unapproved drug when a seller's website showed it was meant for people.20

What has not been shown

The sources reviewed here include no controlled trial in healthy adults showing fat loss, muscle gain, better sleep, skin or hair changes, or slower aging with sermorelin itself.4 In the 6-week study of older men, weight and DEXA fat did not move.5 Detail by goal: Sermorelin and weight loss, Sermorelin and muscle growth and Peptides and men's health.

How strong is the evidence

A 2026 review places sermorelin's human evidence at the level of a past pediatric approval plus phase I/II adult endocrine-testing studies, with no current approval for performance or body-composition use.4 It judges clinically meaningful benefits in healthy people uncertain.4 It also notes that no long-term safety data exist.4

Three limits run through the list above. Groups were small, from 8 to 110 people.111 Adult trials lasted 14 days to 16 weeks, and the chart review covered a mean of 134 days.3615 Several key results come from a related analog or a three-peptide combination, not sermorelin alone.615

Frequently asked questions

What are the benefits of sermorelin for men?

The male data are small and short. In 10 older men, two weeks on the higher tested dose raised GH and IGF-1 to young-adult levels.3 In 11 men aged 64 to 76, six weeks of nightly injections improved 2 of 6 strength measures but did not change muscle or fat mass.5 A related analog, not sermorelin, raised lean mass, libido and well-being in 9 older men over 16 weeks.6

Does sermorelin improve sleep?

No sermorelin trial with sleep outcomes turned up in the sources reviewed here. The closest data come from a 16-week trial of a related GHRH analog in 19 adults aged 55 to 71. Sleep quality, measured by questionnaire, did not change in men or women.6 More on this goal: Peptides and sleep.

Does sermorelin help skin or hair?

Only a related analog has skin data. In 19 adults aged 55 to 71, 16 weeks of [Nle27]GHRH(1-29)-NH2 increased skin thickness, measured with calipers, in both men and women.6 That compound is not sermorelin, and no hair outcomes appear in the sources reviewed here. More on this goal: Peptides and skin and hair.

What happens when you stop sermorelin?

No adult data on stopping turned up in the sources reviewed here. The one finding comes from GH-deficient children: GHRH antibodies that formed during 6 months of treatment had almost disappeared 9 months after it stopped.7 Whether any benefit lasts after stopping was not reported in these studies, and a 1999 review said the effect on final adult height had not been determined.2
