Research goal

Sermorelin and weight loss

No published trial reviewed here shows sermorelin causes weight or fat loss. In the one sermorelin study that measured body fat, 6 weeks of nightly injections in 11 older men changed neither weight nor DEXA fat.1 In a test of 23 women, obesity blunted the growth hormone response to the same peptide.2 This page maps that evidence and makes no promise.

What sources report

Sermorelin raises growth hormone (GH): it binds the GHRH receptor, and that binding leads to GH synthesis and release.59 Whether raising GH this way leads to fat loss is a separate question, answered only by studies that measure fat. The evidence on sermorelin fat loss comes down to the few small studies below, each with its evidence level. The compound itself is covered in What is Sermorelin?, and other compounds for this goal are on Peptides and weight loss.

The direct test: body fat in older men

Evidence level: before-and-after study (n=11). Eleven healthy, non-obese men aged 64 to 76 with low IGF-1 self-injected sermorelin nightly at home for 6 weeks.1 Nighttime GH rose, but IGF-1 did not.1 Weight, BMI, waist-to-hip ratio, DEXA muscle and fat, glucose, insulin and lipids all stayed the same.1

The authors themselves suggested that one dose a night works less well than several doses a day.1 So this study shows one schedule failing to move body fat. It does not test other schedules, longer use or people with obesity.

Evidence level: single-blind trial with a 4-week placebo lead-in (n=19). Ten women and 9 men aged 55 to 71 took [Nle27]GHRH(1-29)-NH2, a related analog that is not sermorelin, nightly for 16 weeks.10 Lean body mass rose in men only, and no other body-composition measure changed.10 Insulin sensitivity improved in men but not in women, while fasting insulin and glucose stayed the same.10

Obesity weakens the response

Evidence level: one-time diagnostic challenge study (n=23). When 14 obese women and 9 normal-weight women received IV GHRF(1-29)NH2, the obese group's mean peak GH was 8.9 mU/L against 28 mU/L in controls.2 That is roughly a third of the normal response. This was a single test, not a treatment trial, but it shows the same peptide getting a weaker pituitary answer in these obese women.2

A chart review of men seeking fat loss

Evidence level: retrospective chart review (n=14). The review screened 105 men on testosterone therapy who wanted more lean mass and less fat, and who were prescribed GHRP-6, GHRP-2 and sermorelin three times daily.11 In the 14 men who met its criteria, mean IGF-1 rose from 159.5 to 239.0 ng/mL.11 Men also taking drugs that lower estrogen or block its receptor had smaller rises.11 Sermorelin was never given alone, and the paper reports no fat result: body measurements were not available for most men at follow-up.20

What the approved GHRH analog's label says

Tesamorelin (Egrifta WR) is a synthetic GHRH analog built on the full 44-amino-acid sequence.3 Its FDA label limits it to reducing excess abdominal fat in HIV-infected adults with lipodystrophy.3 The label also states that it is not indicated for weight loss management because it has a weight neutral effect.3 More: What is Tesamorelin?

How it differs from GLP-1 medicines

Semaglutide is an incretin mimetic, used to control blood sugar in type 2 diabetes and to assist weight loss in certain people.4 Sermorelin works upstream of growth hormone instead.5 The two act through different systems, and the weight data for semaglutide say nothing about sermorelin. Background: What is Semaglutide? and Semaglutide and weight loss.

Blood sugar and appetite

In 110 GH-deficient children treated for up to a year, fasting glucose did not change.12 In older men, fasting glucose did not change over 14 days at each of two doses.13 In the 6-week study, glucose and insulin responses to an oral glucose tolerance test were unchanged.1 For appetite, a 2026 review reports appetite changes, blood sugar disturbance and fluid retention across GH-axis peptides as a group, not for sermorelin alone.6 More on the metabolic side: Peptides and blood sugar.

Who the studies included

The sermorelin fat measurements come from non-obese older men.1 The only study that selected obese adults was the one-time GH test, which enrolled women.2 The analog trial enrolled adults aged 55 to 71, and the chart review enrolled men on testosterone therapy.1011 Women took part only in the GH test and the analog trial, where the 10 women had no lean-mass change.10 No group here was a weight-loss treatment trial in people with overweight or obesity.

Whether IGF-1 follows GH

A 2026 review describes a secondary rise in IGF-1 after the GH rise, but in these studies IGF-1 did not always follow.14 In older men, IGF-1 rose only at the higher of two doses over 14 days.13 With single nightly doses for 6 weeks, GH rose and IGF-1 did not.1 In GH-deficient children, IGF-1 rose at first and then fell back to about pretreatment values within 6 months.15

Online fat-loss claims against the data

Claims that sermorelin trims belly fat meet one direct measurement, and it showed no change in DEXA fat after 6 weeks.1 A 2026 review set the dosing regimens found on fitness and bodybuilding websites beside study regimens to show how widely they differ.14 The same review notes that product content and stacking practices are uncertain in unregulated supply chains.6

Safety notes

A 1999 drug review reported brief facial flushing and injection-site pain as the most common adverse events.16 In one 6-month trial, 39 of 40 children formed GHRH antibodies.15 Full list and warnings: Sermorelin side effects.

What is and is not known

Known, from small studies:

  • Sermorelin raises GH in the short term; after one IV dose in healthy men, GH stayed raised for about 3 hours.17
  • In 11 older men, 6 weeks of nightly injections did not change weight or fat.1
  • In one test of 23 women, obesity blunted the GH response to the same peptide.2
  • Tesamorelin, an FDA-approved GHRH analog, is labeled as having a weight neutral effect.3

Not known:

  • Whether sermorelin causes fat loss in any group. No published trial in these sources shows it, and a 2026 review describes the human evidence for benefits in healthy people as limited and largely indirect.14
  • Long-term safety. No long-term safety data exist.14
  • What a research or compounded product actually contains. Product content and dose are uncertain in unregulated supply chains, and compounded drugs are not FDA-approved.618

Sermorelin's only approvals, a treatment for GH-deficient children and a diagnostic test, never covered weight loss or body composition, and both were withdrawn in 2009.8 A 2026 review confirms there is no current approval for body-composition use.14 Athletes should note that WADA bans it at all times.19

Frequently asked questions

Does sermorelin help with fat loss?

No published trial in the sources reviewed here shows it. In 11 older men, 6 weeks of nightly injections did not change weight, BMI, waist-to-hip ratio or DEXA fat.1 In a test of 14 obese and 9 normal-weight women, obesity blunted the GH response to GRF(1-29).2 Tesamorelin, an FDA-approved GHRH analog, is labeled not for weight loss management because it has a weight neutral effect.3

Is sermorelin like Ozempic?

No. Semaglutide, the drug in Ozempic and Wegovy, is an incretin mimetic used to control blood sugar in type 2 diabetes and to assist weight loss in certain people.4 Sermorelin acts on the growth hormone system instead, through the GHRH receptor.5 In 11 older men, 6 weeks of nightly sermorelin injections did not change weight or fat.1 More on that class: What is GLP-1?

Is sermorelin an appetite suppressant?

No study reviewed here shows that sermorelin suppresses appetite. It works by stimulating growth hormone release through the GHRH receptor, not as an incretin drug like semaglutide.54 A 2026 review lists appetite changes among the reported effects of GH-axis peptides as a group, without saying in which direction or tying them to sermorelin specifically.6

Is tesamorelin the same as sermorelin for fat loss?

No. Both are synthetic GHRH analogs, but tesamorelin is built on all 44 amino acids of GHRH plus a hexenoyl group, while sermorelin is the 29-amino-acid fragment.37 Tesamorelin is FDA-approved to reduce excess abdominal fat in adults with HIV-associated lipodystrophy, and its label says it is not for weight loss management.3 Sermorelin has no current approval.8 See What is Tesamorelin?
