Rybelsus and blood sugar
Rybelsus for type 2 diabetes is an FDA-approved daily tablet that, with diet and exercise, improves blood sugar control in adults.12 In its trial without other diabetes drugs, 77% of adults on 14 mg reached an HbA1c under 7% at 26 weeks, against 31% on placebo.1 This page maps what sources report; it makes no promise for any person.

What sources report
How it acts on blood sugar
FDA groups Rybelsus with the GLP-1 receptor agonists, drugs that mimic GLP-1, a hormone from the intestine.4 FDA says they stimulate insulin release, reduce blood sugar after a meal and slow food moving through the digestive tract.4 The Rybelsus label describes three actions of semaglutide behind its glucose effect: more insulin, less glucagon and slightly slower stomach emptying after meals.1 The first two happen only when blood sugar is high.1
HbA1c in the label trials
Six trials on the label report HbA1c at week 26 in adults with type 2 diabetes.1 The table shows how many points it fell in each group by then.
| Label trial (week 26) | Adults | Who took part | Fall on Rybelsus | Fall on comparator |
|---|---|---|---|---|
| Trial 11 | 703 | Diet and exercise only | 1.2 (7 mg), 1.4 (14 mg) | 0.3 (placebo) |
| Trial 21 | 822 | Taking metformin | 1.3 (14 mg) | 0.9 (empagliflozin 25 mg) |
| Trial 31 | 1,864 | Taking metformin, with or without a sulfonylurea | 1.0 (7 mg), 1.3 (14 mg) | 0.8 (sitagliptin 100 mg) |
| Trial 41 | 711 | Taking metformin, with or without an SGLT2 inhibitor | 1.2 (14 mg) | 1.1 (liraglutide 1.8 mg), 0.2 (placebo) |
| Trial 51 | 324 | Moderate kidney impairment, mean age 70 | 1.0 (14 mg) | 0.2 (placebo) |
| Trial 61 | 731 | Taking insulin, with or without metformin | 0.9 (7 mg), 1.3 (14 mg) | 0.1 (placebo) |
Trial 4 was the label's only comparison with another GLP-1 drug: 14 mg was non-inferior to daily liraglutide 1.8 mg injection.1
Reaching an HbA1c target
In Trial 1, HbA1c started near 8.0%.1 By week 26, an HbA1c under 7% was reached by 69% on 7 mg and 77% on 14 mg, against 31% on placebo.1 Put the other way, 23% on 14 mg did not reach that target by week 26.1
Timing: what the first weeks do and do not show
The 3 mg dose used for days 1 to 30 is a starting dose that the label says is not effective for blood sugar control.1 Daily dosing also takes 4 to 5 weeks to reach steady blood levels, and 14 mg cannot begin before day 61.1 The label's HbA1c results were read at week 26, on 7 mg or 14 mg.1 The full ladder is on Rybelsus dosage.
Low blood sugar in combination
Low blood sugar on Rybelsus is more frequent when it is combined with a sulfonylurea or insulin, and the prescriber may lower those doses when Rybelsus starts.1 In the insulin trial, blood sugar below 54 mg/dL was recorded in 26% on 7 mg and 30% on 14 mg over 52 weeks, against 32% on placebo.1 Rates with Rybelsus alone are on Rybelsus side effects.
Beyond glucose
Since October 2025, the label also covers lowering the risk of major cardiovascular events in adults with type 2 diabetes at high risk.3 The outcome data are in Rybelsus benefits and uses. Body weight fell in the same diabetes trials; those figures are on Rybelsus and weight loss.
What the label says
- Uses: with diet and exercise, to improve blood sugar control in adults with type 2 diabetes, and to reduce the risk of major cardiovascular events in adults with type 2 diabetes at high risk.1
- Doses: 3 mg once daily for 30 days, then 7 mg, then 14 mg from day 61 if more control is needed; 14 mg is the maximum.1 The prescriber sets and adjusts the dose.
- Taking it: one tablet in the morning on an empty stomach, with up to 4 ounces of plain water, at least 30 minutes before food, drink or other oral medicines.1
- Eyes: a rapid improvement in blood sugar can temporarily worsen diabetic retinopathy, and the label asks patients to report vision changes.1
- Boxed warning: thyroid C-cell tumors in rodents; Rybelsus must not be used by anyone with a personal or family history of medullary thyroid carcinoma or with MEN 2.1

Frequently asked questions
Does Rybelsus help with blood sugar?
Yes. Improving blood sugar control in adults with type 2 diabetes, alongside diet and exercise, is the use FDA first approved in 2019.2 The label says semaglutide raises insulin and lowers glucagon when blood sugar is high.1 In a trial of 731 adults already on insulin, adding Rybelsus 14 mg lowered HbA1c by 1.3 points at week 26, against 0.1 on placebo.1 The 3 mg starting dose does not count toward this: the label calls it not effective for blood sugar control.1
Is Rybelsus suitable for treating prediabetes?
Prediabetes is not a labeled use. Rybelsus is FDA approved for adults with type 2 diabetes, to improve blood sugar control with diet and exercise and to lower major cardiovascular risk in those at high risk.1 The label's trials enrolled adults with type 2 diabetes, so their results do not describe people with prediabetes.1 Any use outside the label would be a decision for a prescriber.
How does Rybelsus compare with Januvia and Jardiance?
In 26-week label trials, Rybelsus lowered HbA1c more than both. Against empagliflozin 25 mg (Jardiance) in 822 adults taking metformin, HbA1c fell 1.3 points on 14 mg vs 0.9.1 Against sitagliptin 100 mg (Januvia) in 1,864 adults taking metformin, with or without a sulfonylurea, HbA1c fell 1.0 point on 7 mg and 1.3 on 14 mg, vs 0.8.1 Which drug suits a patient is the prescriber's call.
Does Rybelsus work for type 1 diabetes?
Its approved uses name adults with type 2 diabetes only.1 Until October 2025 the label carried a limitation of use about type 1 diabetes; FDA removed that line in the same action that added the cardiovascular use, which also names type 2 diabetes.3 The trials reported on the label enrolled adults with type 2 diabetes, so they give no type 1 results.1
