Research goal
Wegovy and blood sugar
Wegovy is not FDA-approved to treat type 2 diabetes or to control blood sugar; its approved uses are weight reduction, heart risk reduction and MASH.1 In a 68-week trial of 807 adults with type 2 diabetes, HbA1c fell 1.6 points on Wegovy against 0.4 on placebo.1 Low blood sugar is more likely when it is combined with insulin or a sulfonylurea.1

What sources report
Wegovy for type 2 diabetes is not an approved use, but the label reports two weight trials in adults who had it.1 Both were placebo-controlled phase 3 trials.1
Wegovy trials in adults with type 2 diabetes
- STEP 2 (label Study 3). 807 adults with type 2 diabetes took Wegovy or placebo for 68 weeks.1 HbA1c fell 1.6 points on Wegovy against 0.4 points on placebo, from an average baseline of 8.1%.1 Body weight fell 9.6% against 3.4%.1
- Study 9. 512 adults with type 2 diabetes were followed for 72 weeks.1 Weight changed -13.2% on 7.2 mg, -10.4% on 2.4 mg and -3.8% on placebo.1
In both, average weight loss was smaller than in the matching trials of adults without diabetes: -14.9% in STEP 1, and -18.8% on 7.2 mg in Study 8.1
How semaglutide acts on blood sugar
Semaglutide stimulates insulin release and lowers glucagon release in a glucose-dependent way, meaning the effect depends on the blood sugar level.67 It also causes a minor delay in stomach emptying early after a meal, and together these effects lower blood sugar.67 GLP-1 itself is a hormone released by the intestine that helps lower blood sugar after eating.8
Semaglutide brands approved for type 2 diabetes
The semaglutide medicines with a type 2 diabetes approval are Ozempic injection, and Rybelsus and Ozempic tablets.23 Ozempic's label covers blood sugar control, lowering major cardiovascular events in people with heart disease, and lowering kidney decline and cardiovascular death in people with chronic kidney disease.2 In a 30-week Ozempic trial without other diabetes medicines, HbA1c fell 1.4 points on 0.5 mg and 1.6 points on 1 mg, against 0.1 on placebo.9 Ozempic's weekly doses run from 0.25 mg to a maximum of 2 mg.4
Other GLP-1 receptor agonists approved for type 2 diabetes include liraglutide (Victoza), dulaglutide (Trulicity) and exenatide (Byetta, Bydureon BCise).10111213 Tirzepatide (Mounjaro) acts on both the GIP and GLP-1 receptors and is also approved for type 2 diabetes.14 The compounds people look up for this goal are mapped on Peptides and blood sugar.
Heart and kidney evidence in diabetes
SELECT, the trial behind Wegovy's heart approval, enrolled only adults without diabetes.1 In type 2 diabetes, SUSTAIN-6 tested semaglutide 0.5 mg or 1 mg weekly in 3,297 people for 104 weeks.15 The main cardiovascular outcome occurred in 6.6% against 8.9% on placebo (hazard ratio 0.74), but retinopathy complications were more frequent with semaglutide (hazard ratio 1.76).15 In FLOW, among 3,533 people with type 2 diabetes and chronic kidney disease, semaglutide 1 mg weekly cut the risk of the main kidney outcome by 24%.16 Those doses sit in Ozempic's range, below Wegovy's maintenance dose.41
What the label says
- Approved uses. Weight reduction, heart risk reduction and MASH; treating type 2 diabetes is not among them.1
- Low blood sugar. In adults with type 2 diabetes, clinically significant low blood sugar occurred in 6.2% on Wegovy against 2.5% on placebo.1 The risk is higher alongside insulin or a sulfonylurea.1
- Diabetic eye disease. Over 2 years in type 2 diabetes, retinopathy complications affected 3% on semaglutide 0.5 mg or 1 mg against 1.8% on placebo.1 In Wegovy's own weight trial in type 2 diabetes, diabetic retinopathy was reported in 4% against 2.7% on placebo.17 Both the Wegovy and Ozempic labels add that rapid improvement in blood sugar has been linked to temporary worsening of diabetic retinopathy, and that people with a history of it should be monitored.118
- Tablets. Wegovy tablets have not been studied for weight reduction in people with type 2 diabetes.1
- Stomach emptying. Wegovy slows stomach emptying, and it is not recommended in severe gastroparesis, a condition of very slow stomach emptying.15
- Kidneys. Acute kidney injury, at times needing dialysis, has been reported, mainly after fluid loss from nausea, vomiting or diarrhea.1 The Medication Guide advises drinking fluids to lower the chance of dehydration.1
- No doubling up. The label does not recommend using Wegovy with other semaglutide products or any other GLP-1 receptor agonist.1
- Who decides. The prescriber sets each Wegovy dose step and may lower an insulin or sulfonylurea dose to reduce the risk of low blood sugar.15

Frequently asked questions
Is there a diabetic version of Wegovy?
Not under the Wegovy name.1 The semaglutide brands approved for type 2 diabetes are Ozempic, a weekly injection, and Rybelsus and Ozempic tablets, taken daily.23 Ozempic's weekly doses top out at 2 mg, while Wegovy's maintenance dose is 2.4 mg and can reach 7.2 mg for adult weight reduction.41 Wegovy itself is approved for weight, heart risk and MASH, not for blood sugar control.1
What is better for diabetics, Wegovy or Ozempic?
The sources compare approvals, not a winner. Ozempic is the semaglutide injection approved for adults with type 2 diabetes, for blood sugar control and for lowering heart and kidney risks.2 Wegovy is not approved to treat diabetes, though in a 68-week trial of 807 adults with type 2 diabetes it lowered HbA1c by 1.6 points against 0.4, and weight by 9.6% against 3.4%.1 The Wegovy label does not recommend using them together.1 The choice belongs to the prescriber.1 The two brands are set side by side in Ozempic vs Wegovy.
Can Wegovy cause low blood sugar?
Yes. In the label's trial of adults with type 2 diabetes, 6.2% on Wegovy had clinically significant low blood sugar (below 54 mg/dL), against 2.5% on placebo.1 The risk rises when Wegovy is combined with insulin or a sulfonylurea, and GLP-1 labels say the dose of those medicines may need to be lowered.15 Severe low blood sugar is also one of the reported effects of a GLP-1 overdose.1
Can people with type 2 diabetes use Wegovy?
The label studied Wegovy in adults with type 2 diabetes and gives warnings specific to them.1 Low blood sugar is more likely with insulin or a sulfonylurea, and people with diabetes are asked to report vision changes because of a diabetic eye disease warning.1 Wegovy tablets have not been studied for weight reduction in this group.1 The label does not recommend combining Wegovy with other semaglutide products, such as Ozempic or Rybelsus, or with any other GLP-1 receptor agonist.123 Whether it fits a person with diabetes is a prescriber decision.1
