Benefits and uses

Rybelsus benefits and uses

Rybelsus is used in adults with type 2 diabetes for two FDA-approved purposes: blood sugar control alongside diet and exercise, and lowering the risk of major cardiovascular events in those at high risk.12 Its label trials report lower HbA1c on 7 mg and 14 mg, and fewer major cardiovascular events on 14 mg than on placebo.1

Approved uses

  • Blood sugar control, approved in 2019: FDA's original approval covered Rybelsus, together with diet and exercise, to improve glycemic control in adults with type 2 diabetes.3
  • Cardiovascular risk reduction, approved in 2025: on October 17, 2025, FDA added a use to reduce the risk of major adverse cardiovascular events, meaning cardiovascular death, non-fatal heart attack or non-fatal stroke, in adults with type 2 diabetes who are at high risk of them.2

Both uses are for adults with type 2 diabetes.1 Anything else people search for sits outside the label, as the evidence table below shows.

What the trials found

Every result here comes from a randomized trial reported on the label, in adults with type 2 diabetes.1 They are group averages, not a forecast for any one person.

Blood sugar

Six label trials enrolled 324 to 1,864 adults each and reported HbA1c at week 26. Across them, HbA1c fell 0.9 to 1.4 points on Rybelsus; in the four with a placebo group, it fell 0.1 to 0.3 points on placebo.1 Rybelsus 14 mg lowered HbA1c more than empagliflozin and sitagliptin tablets, and about as much as daily liraglutide injection.1 Trial-by-trial numbers are in Rybelsus and blood sugar.

Heart and blood vessels

The cardiovascular outcomes trial enrolled 9,650 adults aged 50 or older with type 2 diabetes plus heart or blood vessel disease, chronic kidney disease or both, and followed them for a median of about 49.6 months.1

Outcome Rybelsus 14 mg Placebo Hazard ratio
First major cardiovascular event1 12.0% 13.8% 0.86 (95% CI 0.77 to 0.96)
Non-fatal heart attack1 4.0% 5.2% 0.74

For cardiovascular death (hazard ratio 0.93) and non-fatal stroke (hazard ratio 0.88), the confidence intervals crossed 1, so neither was clearly lowered on its own.1

A separate cardiovascular safety trial followed 3,183 adults with type 2 diabetes and high cardiovascular risk for a median of 16 months. Major cardiovascular events occurred in 3.8% on 14 mg and 4.8% on placebo (hazard ratio 0.79, 95% CI 0.57 to 1.11), and Rybelsus met the trial's non-inferiority test against placebo.1 Because that interval includes 1, the safety trial alone did not show a reduction; the larger outcomes trial did.1

Weight

Weight fell in the diabetes trials as well, but their primary measure was HbA1c, and weight loss is not an approved use.1 The figures, and the semaglutide tablet that is approved for weight reduction, are on Rybelsus and weight loss.

Frequently asked questions

What is Rybelsus used for?

Rybelsus has two FDA-approved uses, both in adults with type 2 diabetes. It is used with diet and exercise to improve blood sugar control, its original use from 2019.3 Since October 2025, it is also used to lower the risk of major cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke) in those at high risk.2 It is not approved for weight loss, and the prescriber decides whether it fits a given patient.1

Is Rybelsus effective?

On average, yes, for its approved uses. In six label trials, HbA1c measured at week 26 fell 0.9 to 1.4 points on Rybelsus, against 0.1 to 0.3 points in the four trials that had a placebo group.1 In the outcomes trial that followed 9,650 high-risk adults for a median of about 49.6 months, major cardiovascular events occurred in 12.0% on 14 mg and 13.8% on placebo.1 These are group results from adults with type 2 diabetes, not a prediction for any one person.

Does Rybelsus lower the risk of heart attack?

In the label's cardiovascular outcomes trial of 9,650 high-risk adults with type 2 diabetes, non-fatal heart attacks occurred in 4.0% on Rybelsus 14 mg vs 5.2% on placebo (hazard ratio 0.74).1 FDA's October 2025 approval covers cardiovascular death, non-fatal heart attack and non-fatal stroke as one combined risk.2 For cardiovascular death and non-fatal stroke on their own, the trial's confidence intervals crossed 1.1

Does Rybelsus help people with kidney disease?

In a 26-week trial of 324 adults with moderate kidney impairment (eGFR 30 to 59, mean age 70), HbA1c fell 1.0 point on 14 mg vs 0.2 on placebo.1 That is a blood sugar result in people with weaker kidneys, not a kidney-protection result. Lowering the risk of kidney decline is a use on the Ozempic injection label, not on the Rybelsus label.41 The Rybelsus dose is the same with or without kidney problems.1
