Research goal
Tirzepatide and blood sugar
Tirzepatide is approved as Mounjaro to improve blood sugar control in adults and children 10 and older with type 2 diabetes.1 In 40-week trials, HbA1c dropped by roughly 1.9 to 2.3 percentage points, more than with semaglutide 1 mg.23 Over 3 years in adults with obesity and prediabetes, 1.3% were diagnosed with type 2 diabetes versus 13.3% on placebo.45

What sources report
How it acts on blood sugar
- Insulin and glucagon: tirzepatide boosts first- and second-phase insulin release and lowers glucagon, both only when glucose is raised.6 After 28 weeks, 15 mg cut fasting glucagon by 28% and after-meal glucagon by 43%, with no change on placebo.7
- Insulin sensitivity: in a clamp study in people with type 2 diabetes, insulin sensitivity improved after 28 weeks of treatment.8
- Drug class: MedlinePlus places it among the incretin mimetics, which help the pancreas release the right amount of insulin when blood sugar is high.9
- Two receptors: it acts on both the GIP and GLP-1 receptors, the targets of two gut hormones.10 In cell studies it bound the GIP receptor about as strongly as natural GIP.11

SURPASS-1: tirzepatide on its own
SURPASS-1 compared tirzepatide 5, 10 or 15 mg with placebo for 40 weeks.122 Its 478 adults had diabetes not controlled by diet and exercise.12 HbA1c fell 1.87%, 1.89% and 2.07% on the three doses (20, 21 and 23 mmol/mol), versus a 0.04% rise on placebo.2 Body weight fell 7.0 to 9.5 kg.19 No clinically significant (below 54 mg/dL) or severe low blood sugar was reported with tirzepatide.20
SURPASS-2: against semaglutide
SURPASS-2 added tirzepatide or semaglutide 1 mg to metformin in 1,879 adults for 40 weeks.133 HbA1c fell 2.01, 2.24 and 2.30 percentage points with 5, 10 and 15 mg of tirzepatide, versus 1.86 with semaglutide.3 Every tirzepatide dose was noninferior and superior to semaglutide, and weight loss was 1.9 to 5.5 kg greater.21 Nausea occurred in 17% to 22% on tirzepatide versus 18% on semaglutide.22
Children and teens
SURPASS-PEDS enrolled 99 patients aged 10 and older with type 2 diabetes.14 The 30-week randomized trial had a 22-week open-label extension.14 Both 5 mg and 10 mg once weekly lowered HbA1c significantly more than placebo.15 This trial supports the pediatric use added to the Mounjaro label in December 2025.1423
Preventing type 2 diabetes
Among 1,032 adults with obesity and prediabetes in SURMOUNT-1, about 3 years of treatment led to 1.3% being diagnosed with type 2 diabetes, versus 13.3% on placebo.45 The hazard ratio was 0.07.5 Diabetes prevention is not one of the labeled uses.124 Weight results from the same trial are on Tirzepatide and weight loss.
A model estimate for the starting dose
The Mounjaro label includes an exposure-response analysis built from phase 2 and phase 3 data.25 For 2.5 mg weekly, it projects an HbA1c drop of about 1.8% and a fasting glucose drop of about 43 mg/dL at 52 weeks.25 That is a projection rather than a measured result, and the label still treats 2.5 mg as the starting step.2526
Heart outcomes
SURPASS-CVOT compared tirzepatide (up to 15 mg) with dulaglutide 1.5 mg in 13,299 adults with type 2 diabetes and established cardiovascular disease.16 Over a median of 210 weeks, 12.2% of people on tirzepatide and 13.1% on dulaglutide had a major cardiovascular event, a hazard ratio of 0.92.1817 That met the test for noninferiority, not for superiority.18 FDA added a heart risk indication to the Mounjaro label in August 2026.2723
What the label says
- Approved uses: Mounjaro is approved, with diet and exercise, to improve blood sugar control in adults and children 10 and older with type 2 diabetes.1 It is also approved to lower the risk of cardiovascular death, heart attack or stroke in high-risk adults with type 2 diabetes.27
- Dosing: it starts at 2.5 mg once weekly.26 If more control is needed, the dose can rise by 2.5 mg once the current dose has been taken for at least 4 weeks.26 The ceiling is 15 mg in adults and 10 mg in children.28 The prescriber sets the dose; see Tirzepatide dosage.
- Who should not use it: people with a personal or family history of medullary thyroid carcinoma or MEN 2, or a known serious allergy to tirzepatide.29
- Low blood sugar: the risk, including severe low blood sugar, rises when it is combined with insulin or a sulfonylurea.30
- Stomach and bowel effects: in the diabetes trials they affected 37% to 44% on tirzepatide versus 20% on placebo, with nausea in 12% to 18% versus 4%.3132 Most nausea, vomiting and diarrhea occurred while the dose was being raised.33
- Pancreatitis: confirmed cases occurred in 0.23 patients per 100 years of exposure, versus 0.11 on comparators.34
- Eyes: rapid improvement in blood sugar has been linked to temporary worsening of diabetic retinopathy.35 People with a history of it should be monitored.35
- Not recommended: in severe gastroparesis.36 Safety and effectiveness are not established under age 10.37
- European Union: Mounjaro is used with diet and physical activity for type 2 diabetes in adults, adolescents and children 10 and older.38

Research-labeled tirzepatide
The results on this page come from trials of the approved injectable. No FDA application covers research-labeled tirzepatide.39 FDA warns that research-labeled GLP-1 products reaching consumers are of unknown quality, so none of these glucose results should be read as applying to them.40

Frequently asked questions
Is tirzepatide approved for type 2 diabetes?
Yes. FDA approved it as Mounjaro on May 13, 2022.41 The current label covers blood sugar control, with diet and exercise, in adults and children 10 years and older with type 2 diabetes, and lowering cardiovascular risk in high-risk adults with type 2 diabetes.127 It is a prescription medicine.42 In the EU, Mounjaro was authorised in September 2022.43
How much does tirzepatide lower A1C?
In 40-week phase 3 trials, HbA1c fell 1.87% to 2.07% with tirzepatide alone in 478 adults.122 When it was added to metformin in 1,879 adults, HbA1c fell 2.01 to 2.30 percentage points, versus 1.86 with semaglutide 1 mg.133 Results vary by person, and the dose is set by the prescriber under the label schedule.26
Can tirzepatide cause low blood sugar?
On its own, the risk looked low: SURPASS-1 reported no clinically significant (below 54 mg/dL) or severe low blood sugar with tirzepatide.20 Its insulin effect is glucose-dependent, meaning it acts when glucose is raised.6 The risk rises with insulin or a sulfonylurea, and in a weight-reduction trial of people with type 2 diabetes, low blood sugar occurred in 4.2% versus 1.3% on placebo.3044
Can tirzepatide prevent type 2 diabetes?
It is not approved for that. In SURMOUNT-1's three-year analysis of 1,032 adults with obesity and prediabetes, 1.3% on tirzepatide were diagnosed with type 2 diabetes versus 13.3% on placebo.45 That is a trial result in one group, not a labeled use. The labels cover blood sugar control in type 2 diabetes, heart risk, weight management and sleep apnea.1272445
