Research goal
Semaglutide and blood sugar
Semaglutide for diabetes is FDA approved as Ozempic injection and as Rybelsus and Ozempic tablets, to improve blood sugar control in adults with type 2 diabetes.12 In a 30-week Ozempic trial, HbA1c fell 1.4 to 1.6 points vs 0.1 on placebo.3 These are group averages, and whether it suits a patient is a prescriber's decision.

What sources report
How it acts on blood sugar
Semaglutide stimulates insulin release and lowers glucagon release in a glucose-dependent way, and it causes a minor delay in stomach emptying soon after a meal; together these lower blood sugar.14 Appetite effects and the molecule's design are covered on What is Semaglutide?.
HbA1c and weight on the Ozempic label
| Trial on the Ozempic label3 | HbA1c change | Body weight change |
|---|---|---|
| 30 weeks, semaglutide 0.5 mg weekly | -1.4 points | -3.8 kg |
| 30 weeks, semaglutide 1 mg weekly | -1.6 points | -4.7 kg |
| 30 weeks, placebo | -0.1 points | -1.2 kg |
| 40 weeks, semaglutide 1 mg weekly | -1.9 points | -5.6 kg |
| 40 weeks, semaglutide 2 mg weekly | -2.1 points | -6.4 kg |
The 30-week trial tested semaglutide alone (monotherapy) in type 2 diabetes. In the 40-week trial, the weight difference between 2 mg and 1 mg was not statistically significant.3
Heart outcomes in type 2 diabetes
SUSTAIN-6 followed 3,297 people with type 2 diabetes for 104 weeks on semaglutide 0.5 or 1.0 mg weekly or placebo.16 The primary cardiovascular outcome occurred in 6.6% vs 8.9% (hazard ratio 0.74), and nonfatal stroke in 1.6% vs 2.7%.16 Retinopathy complications were more frequent with semaglutide (hazard ratio 1.76).16
Kidney outcomes
FLOW gave semaglutide 1.0 mg weekly or placebo to 3,533 people with type 2 diabetes and chronic kidney disease.15 It stopped early after a planned interim analysis, at a median follow-up of 3.4 years, with a 24% lower risk of the primary kidney outcome on semaglutide (hazard ratio 0.76).15
Tablets for type 2 diabetes
Semaglutide for diabetes also comes as once-daily tablets: Rybelsus (3, 7 and 14 mg) and Ozempic tablets (1.5, 4 and 9 mg).9 Their first doses, 3 mg and 1.5 mg, are starting doses that the labels say are not effective for glucose control.217 The most common side effects on the shared tablet label are nausea, abdominal pain, diarrhea, decreased appetite, vomiting and constipation.17
Weight change in type 2 diabetes
In the Wegovy label's two 72-week studies, adults with obesity and type 2 diabetes lost less weight than adults with obesity but without type 2 diabetes in the other study: -10.4% vs -15.5% on 2.4 mg weekly, and -13.2% vs -18.8% on 7.2 mg.18 The full weight data are on Semaglutide and weight loss.
Limits of this evidence
All of these figures are group averages from trials of semaglutide medicines.316 The sources used here give no HbA1c results for the tablets and do not address type 1 diabetes, so this page covers neither. None of them concerns research-labeled semaglutide; FDA says unapproved versions of GLP-1 drugs do not undergo its review for safety, effectiveness and quality.19
What the label says
Ozempic injection is approved for adults with type 2 diabetes for three uses.1
- Blood sugar control, alongside diet and exercise.1
- Lowering the risk of major cardiovascular events (cardiovascular death, nonfatal heart attack or nonfatal stroke) in those with established cardiovascular disease.1
- Lowering the risk of sustained kidney function decline, end-stage kidney disease and cardiovascular death in those with chronic kidney disease.1
Rybelsus and Ozempic tablets share one label.2 It covers blood sugar control in adults with type 2 diabetes, and lowering the risk of major cardiovascular events in high-risk adults with type 2 diabetes.2
The prescriber sets the dose. Ozempic injection starts at 0.25 mg weekly for 4 weeks, then 0.5 mg, with increases to 1 mg and then 2 mg, the maximum, if more glucose control is needed; the maintenance dose for chronic kidney disease is 1 mg.8 Injection and tablet schedules side by side are on Semaglutide dosage.
Label warnings that matter most in diabetes:
- Low blood sugar: combining semaglutide with insulin or an insulin secretagogue raises the risk, including severe low blood sugar.12
- Diabetic eye disease: retinopathy complications occurred in 3.0% vs 1.8% in a 2-year Ozempic trial, and rapid improvement in blood sugar can temporarily worsen diabetic retinopathy.20
- Severe gastroparesis: Ozempic is not recommended.20
- Boxed warning: thyroid C-cell tumors in rodents, with human relevance unknown.21
FDA has also found counterfeit Ozempic in the US drug supply chain, and it advises getting Ozempic only with a valid prescription through state-licensed pharmacies.22 The complete list of side effects is on Semaglutide side effects. None of this covers research-labeled semaglutide, which no approved application covers and which FDA treats as an unapproved new drug when it is intended for people.10

Frequently asked questions
What happens if you take semaglutide without diabetes?
Wegovy's weight and heart indications do not require diabetes, and the SELECT trial enrolled 17,604 adults with heart disease and overweight or obesity but no history of diabetes.45 The Wegovy label lists low blood sugar as a common side effect only in patients with type 2 diabetes; digestive side effects are the most common for everyone.6 Use without diabetes is still prescription only and set by a prescriber.7
Is Ozempic the same as semaglutide?
Ozempic is one brand of semaglutide: an FDA-approved prescription injection (0.25 to 2 mg weekly) and tablet for adults with type 2 diabetes.189 Wegovy and Rybelsus contain the same molecule at different doses and for different uses.49 A research vial labeled semaglutide is not Ozempic; FDA treats research-labeled semaglutide sold for human use as an unapproved new drug.1011 See Ozempic vs Semaglutide.
Does semaglutide cause low blood sugar?
It can. In a Wegovy trial in people with type 2 diabetes, clinically significant low blood sugar (below 54 mg/dL) occurred in 6.2% vs 2.5% on placebo.13 The labels warn that the risk rises, including severe episodes, when semaglutide is used with insulin or an insulin secretagogue.12 The Ozempic label describes its effects on insulin and glucagon as glucose-dependent.14
Does semaglutide protect the kidneys in diabetes?
In the FLOW trial of 3,533 people with type 2 diabetes and chronic kidney disease, semaglutide 1.0 mg weekly lowered the risk of the main kidney outcome by 24% compared with placebo.15 Median follow-up was 3.4 years.15 The Ozempic label now includes lowering the risk of sustained kidney function decline, end-stage kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease.1
