Dosage information

Rybelsus dosage

The Rybelsus label sets a three-step schedule: 3 mg once daily for days 1 to 30, a starting dose not effective for blood sugar control; 7 mg from day 31; and 14 mg, the maximum, from day 61 if more control is needed.1 Only one tablet is taken each day,1 and the prescriber sets each step.

Label dosing schedule

These are the FDA label doses for Rybelsus, a prescription medicine. They are published dosing information, not a dose for any reader: the prescriber decides when to start, whether to move from 7 mg to 14 mg, and how to adjust other diabetes medicines.

Days of treatment Once-daily dose What the label says
1 to 30 3 mg Starting (initiation) dose; not effective for blood sugar control1
31 to 60 7 mg The dose is increased on day 311
61 onward 7 mg or 14 mg Stay on 7 mg if blood sugar control is enough; increase to 14 mg if more is needed. 14 mg is the maximum.1

The label says to follow this step-up schedule to reduce the risk of stomach and gut side effects.1 It allows only one tablet per day, so each step uses its own tablet strength rather than a combination of tablets.1

The 3 mg starting dose

The 3 mg tablet covers days 1 to 30 only. The label calls this the initiation phase and says that at this dose Rybelsus is not effective for blood sugar control.1 Starting low is part of the step-up meant to reduce stomach and gut side effects.1 On day 31 the dose goes up to 7 mg.1 Like the other strengths, the 3 mg tablet is white to light yellow and oval, stamped with its strength on one side and "novo" on the other, and it comes in a bottle of 30.1 The switching rules below also rule out moving to Ozempic tablets during these first 30 days.1

How each dose is taken

Each tablet is taken in the morning on an empty stomach with up to 4 ounces of plain water, swallowed whole, followed by a wait of at least 30 minutes before any food, drink or other oral medicine.1 In label studies, the amount of water and the length of the wait changed how much semaglutide was absorbed.1 The absorption data are explained in the Rybelsus profile.

Special situations on the label

  • Kidney or liver problems: the recommended dose is the same as for people with normal kidney or liver function.1 One label trial tested 14 mg in 324 adults with moderate kidney impairment (eGFR 30 to 59, mean age 70) over 26 weeks.1
  • Men and women: the label has no separate dosing by sex, and no clinically significant differences in semaglutide levels were seen between men and women.1
  • With insulin or a sulfonylurea: when Rybelsus is started, the prescriber may lower the dose of these drugs to reduce the risk of low blood sugar.1
  • Under 18: the label says safety and effectiveness have not been established.1

Switching between Rybelsus and Ozempic tablets

Rybelsus and Ozempic tablets share one label, but they are not substitutable mg for mg.1 Ozempic tablets were sold as Rybelsus formulation R2 until FDA approved the name change in January 2026.5 A larger share of each Ozempic tablet dose is absorbed (about 1% to 2% vs 0.4% to 1% for Rybelsus), and its strengths are lower.1 FDA approved the rules for switching between the two formulations in December 2024, together with the newer tablet.2

Rybelsus, once daily Matching Ozempic tablet, once daily
7 mg 4 mg1
14 mg 9 mg1
  • Timing: switching is only for patients past the first 30 days. The new product starts the day after the last dose of the old one.1
  • Exposure: in healthy volunteers, semaglutide blood levels were similar at these matched doses.1
  • Starting fresh on Ozempic tablets follows its own ladder: 1.5 mg once daily for 30 days, then 4 mg, with an option to increase to 9 mg.3

Go back to the Rybelsus profile for the full overview. Rybelsus side effects covers what the step-up is meant to reduce, and Rybelsus benefits and uses shows what 7 mg and 14 mg achieved in trials. Weekly injection schedules are in Ozempic dosage, and schedules across the drug class are in GLP-1 dosage.

Frequently asked questions

What is Rybelsus 3 mg used for?

The 3 mg tablet is only the starting dose for the first 30 days of treatment.1 The label calls it the initiation dose and says it is not effective for blood sugar control.1 On day 31 the dose rises to 7 mg, and the label's trial results are for 7 mg and 14 mg.1 Starting low is meant to reduce stomach and gut side effects.1

What if I miss a dose of Rybelsus?

The label says to skip the missed dose and take the next one the following day.1 It does not call for making up the dose: no more than one tablet is taken in a day, at any strength.1 The label gives no rule for several missed days in a row, so that is a question for the prescriber.

Can I switch from Ozempic injection to Rybelsus?

The label covers one case. People on the 0.5 mg weekly Ozempic injection may switch, starting Rybelsus at 7 mg or 14 mg once daily one week after the last 0.5 mg injection.1 The label's only other switching rules cover Rybelsus and Ozempic tablets. It gives no rule for other injection doses or for moving from Rybelsus to the injection, so those choices rest with the prescriber.1

What is the highest dose of Rybelsus?

The label's maximum is 14 mg once daily, used from day 61 only if 7 mg does not give enough blood sugar control.1 Higher-milligram semaglutide tablets exist, but they are separate products: Wegovy tablets, approved in December 2025, come in strengths up to 25 mg.4 Even Rybelsus and Ozempic tablets, which share one label, are not substitutable mg for mg.1
