Ozempic and weight loss
Ozempic is not approved for weight loss; its label covers adults with type 2 diabetes.1 Weight still fell in its diabetes trials, by an average of 3.2 to 6.4 kg (about 7 to 14 lb) over 30 to 56 weeks.1 The semaglutide brand FDA approved for weight reduction is Wegovy, at higher doses.45 This page reports what sources found on Ozempic for weight loss and makes no promise.

What sources report
Weight loss is not an Ozempic use on the label, but its diabetes trials measured weight alongside blood sugar.1 Those results, and a few related trials, are below.
Weight change in the Ozempic trials
These are average weight changes from the label, in adults with type 2 diabetes.1
| Trial (evidence level) | Length | Ozempic | Comparison |
|---|---|---|---|
| Ozempic alone (double-blind, 388 adults)1 | 30 weeks | Down 3.8 kg (about 8 lb) on 0.5 mg and 4.7 kg (about 10 lb) on 1 mg | Placebo: down 1.2 kg |
| 2 mg against 1 mg (double-blind, 961 adults)1 | 40 weeks | 2 mg: down 6.4 kg (about 14 lb) | 1 mg: down 5.6 kg (about 12 lb) |
The label reports the weight gap between 2 mg and 1 mg as not statistically significant.1 The published SUSTAIN FORTE report of that trial gives 6.9 kg on 2 mg and 6.0 kg on 1 mg in its analysis that assumes no one stopped treatment or took rescue medication, and 6.4 kg against 5.6 kg when everyone randomized is counted.8
STEP 2: an Ozempic dose against the Wegovy dose
STEP 2 was a 68-week phase 3 trial of 1,210 adults with type 2 diabetes and overweight or obesity, all in a lifestyle program.23 It compared semaglutide 1.0 mg, an Ozempic dose, with 2.4 mg, the Wegovy dose, and with placebo.23 The registry's observed average weight loss was 7.2% on 1.0 mg, 9.9% on 2.4 mg and 3.3% on placebo.3 The journal report's estimated weight loss was 9.6% on 2.4 mg against 3.4% on placebo.2
Why weight changes on semaglutide
FDA describes GLP-1 receptor agonists as slowing the movement of food through the digestive tract, which can help a person feel full longer, and notes that GLP-1 receptors sit in brain areas that regulate appetite.13 The Ozempic label says semaglutide slows stomach emptying, and MedlinePlus says it may decrease appetite and cause weight loss.17 The Wegovy label adds that more of the weight lost on semaglutide is fat mass than lean mass.4
Compared with tirzepatide
In SURPASS-2, an open-label phase 3 trial of 1,879 adults with type 2 diabetes over 40 weeks, tirzepatide 5, 10 and 15 mg lowered weight by 1.9, 3.6 and 5.5 kg more than semaglutide 1 mg.6 Eli Lilly, which makes tirzepatide, funded the trial, and Ozempic 2 mg was not tested.6 The tirzepatide brands are covered in Mounjaro and weight loss and What is Zepbound?.
What happens after stopping
The sources here do not follow weight after people stop Ozempic at its diabetes doses. The closest evidence comes from the 2.4 mg Wegovy dose in adults with obesity or overweight. In STEP 4, a phase 3a trial that randomized 803 adults without diabetes, people switched to placebo after 20 weeks on semaglutide regained 6.9% of body weight by week 68, while those who stayed on it lost a further 7.9%.9 In the STEP 1 extension (327 adults), participants regained two-thirds of their earlier weight loss within a year of stopping, and for most measures, heart and metabolic improvements drifted back toward where they started.10
FDA's warning on unapproved weight-loss products
FDA's page on unapproved GLP-1 drugs used for weight loss says compounded and other unapproved versions do not go through FDA review for safety, effectiveness and quality.12 It counted 990 adverse event reports linked to compounded semaglutide as of May 31, 2026.12 FDA has also warned sellers of semaglutide falsely labeled 'for research purposes' or 'not for human consumption', and calls those products of unknown quality.12
What the label says
The Ozempic label lists three uses, all for adults with type 2 diabetes: blood sugar control, and lowering heart and kidney risks in some of them.1 Weight loss is not one of them.1 FDA has approved Wegovy (semaglutide) and Zepbound (tirzepatide), not Ozempic, for weight reduction.54 Wegovy's usual maintenance dose is 2.4 mg once a week, up to 7.2 mg, against Ozempic's 2 mg maximum.41
On its own label, Ozempic starts at 0.25 mg once a week for 4 weeks, then 0.5 mg, and may rise to 1 mg and then 2 mg if more blood sugar control is needed; 1 mg is the maintenance dose for its kidney use.1 The prescriber sets each step. It carries a boxed warning on thyroid C-cell tumors seen in rodents, and its most common side effects are nausea, vomiting, diarrhea, stomach pain and constipation.1

Frequently asked questions
How long will it take to lose 20 lbs on Ozempic?
The label gives no weight-loss timeline, since weight loss is not an approved use.1 In its diabetes trials, average weight loss on Ozempic ranged from 3.2 to 6.4 kg (about 7 to 14 lb) over 30 to 56 weeks, so no trial average reached 20 lb.1 In the 30-week trial of Ozempic alone it was 3.8 kg on 0.5 mg and 4.7 kg on 1 mg, and every course starts with 4 weeks at a 0.25 mg starting dose.1 In STEP 2, adults with type 2 diabetes on 1.0 mg lost 7.2% of body weight on average over 68 weeks.3
Is Wegovy stronger than Ozempic?
Both contain semaglutide, so the difference is dose and label. Wegovy's usual maintenance dose is 2.4 mg once a week and can go up to 7.2 mg, while Ozempic tops out at 2 mg.41 In STEP 2, adults with type 2 diabetes and overweight or obesity lost 9.9% of body weight on average on 2.4 mg and 7.2% on 1.0 mg over 68 weeks.3 Wegovy is approved for weight reduction; Ozempic is not.51
Does Ozempic help you lose belly fat?
The Ozempic label reports total body weight, not belly fat.1 The Wegovy label says semaglutide lowers body weight with more fat mass lost than lean mass.4 In SURMOUNT-5, a 72-week, open-label phase 3b trial of 751 adults with obesity but without type 2 diabetes, waist size fell by 13.0 cm on average with semaglutide at the highest tolerated Wegovy dose (1.7 or 2.4 mg), against 18.4 cm with tirzepatide.11 No trial in these sources measured belly fat at Ozempic doses.
Can Ozempic and Wegovy be used together?
It is not recommended. The Wegovy label says using it with other semaglutide-containing products, which include Ozempic, or with any other GLP-1 receptor agonist is not recommended.4 Both products contain the same active ingredient, semaglutide, from the same maker, Novo Nordisk.14 Of the two, only Wegovy is approved for weight reduction.51
