Key differences
Same drug, same maker, same disease: both are for adults with type 2 diabetes.12 The split is in how the drug enters the body, and it shows up in the dose, the daily routine and parts of the label.
Weekly injection or daily tablet
Ozempic injection goes under the skin of the abdomen, thigh or upper arm once a week, on the same day, at any time and with or without food.1 Rybelsus is swallowed every morning on an empty stomach with up to 4 ounces of plain water, followed by at least 30 minutes without food, drink or other pills.2
Only about 0.4% to 1% of a Rybelsus dose reaches the bloodstream, against 89% of an injected dose.21 The tablet is dosed at up to 14 mg a day, while the injection tops out at 2 mg a week.21
Missed doses work differently. A missed Ozempic injection can still be given within 5 days; after that it is skipped.1 A missed Rybelsus tablet is simply skipped, and the next one is taken the following day.2
Moving between Rybelsus and Ozempic
The labels set rules for some switches. In each case the prescriber decides whether and when to make one.
From the injection to a tablet: a person on Ozempic 0.5 mg weekly can switch. One week after the last injection, they start Rybelsus 7 mg or 14 mg daily, or Ozempic tablets 4 mg or 9 mg.27
Between the two tablets: Rybelsus and Ozempic tablets share one label but are not substitutable milligram for milligram.2 After the first 30 days, Rybelsus 7 mg matches Ozempic tablets 4 mg, and Rybelsus 14 mg matches 9 mg.2 The new tablet starts the day after the last dose of the old one, and no switch is made during days 1 to 30.2 Ozempic tablets use lower doses for the same exposure: the label estimates 1% to 2% absorption against 0.4% to 1% for Rybelsus, and at matched doses, blood levels in healthy volunteers were similar.2
From a tablet to the weekly injection: neither label gives a switching schedule for this direction, so the prescriber sets the starting dose and timing.12
Approved uses and outcome trials
Both are approved, with diet and exercise, to improve blood sugar control in adults with type 2 diabetes.12 Both also carry a heart indication, from separate trials. In SUSTAIN 6, the trial behind Ozempic's, 3,297 adults with type 2 diabetes and atherosclerotic cardiovascular disease were followed for a median 2.1 years.1 A heart attack, stroke or cardiovascular death struck 6.6% of those on Ozempic and 8.9% of those on placebo.1
Rybelsus gained its heart indication on October 17, 2025.6 In its trial of 9,650 adults aged 50 or older with cardiovascular disease, chronic kidney disease or both, followed a median of about 49.6 months, first major events occurred in 12.0% on 14 mg and 13.8% on placebo (hazard ratio 0.86).2 Non-fatal heart attacks occurred in 4.0% against 5.2%.2
Kidney protection is an Ozempic injection use only. In the FLOW trial of 3,533 adults with type 2 diabetes and chronic kidney disease, Ozempic 1 mg cut the risk of the main kidney and heart outcome by 24% against placebo.1 The tablet label lists only the blood sugar and heart uses.2 Neither product is approved for weight loss.12
Blood sugar results
The Rybelsus label's comparison trials used empagliflozin, sitagliptin and liraglutide, not Ozempic injection.2 So each product's effect comes from its own trials. Ozempic used alone, over 30 weeks in 388 adults, lowered HbA1c by 1.4 points on 0.5 mg and 1.6 on 1 mg, against 0.1 on placebo.1 Rybelsus used alone, over 26 weeks in 703 adults, lowered it by 1.2 points on 7 mg and 1.4 on 14 mg, against 0.3.2 The trials differed in length and patients, so these figures do not rank the two.
Side effects and form-specific warnings
Stomach and gut effects lead on both labels. Nausea affected 15.8% on Ozempic 0.5 mg and 20.3% on 1 mg, against 6.1% on placebo.1 On Rybelsus it affected 11% on 7 mg and 20% on 14 mg, against 6%.2 Stopping treatment because of these effects ran at 3.1% to 3.8% on Ozempic and 4% to 8% on Rybelsus, against 0.4% and 1% on placebo.12
Some differences follow from the form. Injection site reactions, such as discomfort or redness, were reported in 0.2% of Ozempic users in placebo-controlled trials.1 The labels also differ on breastfeeding. The Ozempic label reports no data on semaglutide in human milk.1 The tablet label says breastfeeding is not recommended, because SNAC, the tablet's absorption enhancer, or its breakdown products are present in human milk and could build up in the infant.2
Storage
An unopened Ozempic pen is kept in the refrigerator at 36 to 46°F.1 Once in use, it lasts 56 days at 59 to 86°F or in the refrigerator.1 Rybelsus needs no refrigeration. It is kept at 68 to 77°F in its original closed bottle, which protects the tablets from moisture.2
What stays the same
Both labels carry the boxed warning for thyroid C-cell tumors, which semaglutide caused in mice and rats, and both rule out use with a personal or family history of medullary thyroid carcinoma or with MEN 2.12 Both list pancreatitis, diabetic eye disease, gallbladder problems, kidney injury from dehydration, and low blood sugar when combined with insulin or a sulfonylurea.12 Semaglutide's half-life is about one week in either form, and both labels say to stop at least 2 months before a planned pregnancy.12
In Europe, the medicines regulator's safety committee concluded in June 2025 that NAION, an eye condition that can cause sudden vision loss, is a very rare side effect of semaglutide medicines including Ozempic and Rybelsus, affecting up to 1 in 10,000 people.8 As of October 2026, neither the US Ozempic injection label (revised May 2026) nor the US label for Rybelsus and Ozempic tablets (revised January 2026) lists it.12

The tablet against its active ingredient is covered in Rybelsus vs semaglutide, and the GLP-1 comparisons page lists every pair.
Frequently asked questions
What are the key differences between Ozempic and Rybelsus?
Both contain semaglutide from Novo Nordisk and are approved for adults with type 2 diabetes.12 Ozempic is mainly a weekly injection of 0.25 to 2 mg and also carries a kidney indication; Rybelsus is a daily tablet of 3 to 14 mg taken on an empty stomach.12 Storage differs too: Ozempic pens are refrigerated before first use, while Rybelsus is kept at room temperature.12
Is Rybelsus as strong as Ozempic?
Their labels do not test that directly: the Rybelsus label's comparison trials used other diabetes drugs, not Ozempic injection.2 Each lowered HbA1c against placebo in its own trials, Ozempic by 1.4 to 1.6 points over 30 weeks and Rybelsus by 1.2 to 1.4 points over 26 weeks, in different groups of adults.12 Milligrams are no guide either, since only about 0.4% to 1% of a Rybelsus dose is absorbed.2
Can Ozempic be taken orally?
Yes, since January 30, 2026. On that date FDA approved renaming a once-daily semaglutide tablet, formerly a second Rybelsus formulation, to Ozempic tablets, in 1.5, 4 and 9 mg.54 The weekly Ozempic injection is a separate product with its own doses.1 Ozempic tablets and Rybelsus now share one label but are not swapped milligram for milligram.2
How do you switch from Rybelsus to Ozempic tablets?
The shared tablet label gives a matching table for use after the first 30 days: Rybelsus 7 mg moves to Ozempic tablets 4 mg, and Rybelsus 14 mg to 9 mg.2 The new tablet starts the day after the last Rybelsus dose, and no switch is made during days 1 to 30.2 The label estimates that 1% to 2% of an Ozempic tablet dose is absorbed, against 0.4% to 1% for Rybelsus, and matched doses gave similar blood levels in healthy volunteers.2 The prescriber decides whether and when to switch.
