Rybelsus vs Semaglutide

Rybelsus is one product, a once-daily semaglutide tablet for adults with type 2 diabetes, while semaglutide is the molecule inside it and inside the Ozempic and Wegovy injections and tablets.178

Key differences

Rybelsus is semaglutide in tablet form, combined with an ingredient that helps it get absorbed from the stomach.1 The molecule itself is the same one given by weekly injection as Ozempic and Wegovy.78 What changes with the tablet is the route, the dose scale and the daily routine.

What semaglutide is

Semaglutide is a modified copy of the hormone GLP-1, sharing 94% of its sequence with the human version.3 It stimulates insulin release and lowers glucagon in a glucose-dependent way, and causes a minor delay in stomach emptying after meals.3 Its peptide backbone is made by yeast fermentation, with a C18 fatty diacid joined through a spacer to the lysine at position 26.4 Its molecular weight is 4113.58 g/mol, and its half-life is about one week.41

Absorption and the dose scale

Very little of a swallowed dose reaches the blood: the label estimates 0.4% to 1% for Rybelsus and 1% to 2% for Ozempic tablets, absorbed mostly in the stomach, with blood levels peaking about 1 hour after a dose.1 After an injection under the skin, 89% is absorbed.5 The dose scales sit far apart: Rybelsus maintenance doses are 7 or 14 mg a day, while injected semaglutide runs from 0.25 mg to 7.2 mg a week.113

Both tablets contain salcaprozate sodium (SNAC), an absorption enhancer, and magnesium stearate; Rybelsus also contains microcrystalline cellulose and povidone.1

A strict morning routine

The label is precise about how the tablet is taken. One tablet goes down each morning on an empty stomach, with no more than 4 ounces of plain water, swallowed whole.1 Then comes a wait of at least 30 minutes before any food, drink or other oral medicine.1 In studies of healthy people, more semaglutide was absorbed with 50 mL of water than with 240 mL, and with a longer wait before eating.1 A missed tablet is skipped, and the next one is taken the following day.1

Dose steps

Rybelsus starts at 3 mg once daily for 30 days, a starting dose the label says is not effective for blood sugar control.1 On day 31 it rises to 7 mg, and from day 61 it can go to 14 mg, the highest Rybelsus strength, if more control is needed.1 Only one tablet is taken per day, and the prescriber sets each step.1 Steady blood levels take 4 to 5 weeks of daily dosing to build.1

Three tablet products now share the molecule

"Semaglutide tablets" no longer means only Rybelsus. A second, round formulation was approved on December 9, 2024, and renamed Ozempic tablets on January 30, 2026; it comes in 1.5, 4 and 9 mg.11101 Wegovy tablets of 1.5, 4, 9 and 25 mg were approved on December 22, 2025, for weight reduction and heart risk in adults.98 Rybelsus and Ozempic tablets share one label but are not swapped milligram for milligram, and the label gives a separate switching table.1

What Rybelsus is approved for

Rybelsus is a diabetes medicine. FDA approved it on September 20, 2019, to improve blood sugar control in adults with type 2 diabetes, alongside diet and exercise.21 On October 17, 2025, FDA added a second use: lowering the risk of major cardiovascular events in adults with type 2 diabetes at high risk.12

Rybelsus is not approved for weight loss.1 In its 26-week diabetes trials, weight fell 2.2 to 4.4 kg on Rybelsus 7 or 14 mg and 0.4 to 1.4 kg on placebo.1

Other semaglutide products cover more ground. Ozempic injection adds a kidney indication for adults with type 2 diabetes and chronic kidney disease.7 Wegovy is approved for weight reduction, for heart risk in adults with established cardiovascular disease and obesity or overweight, and for the liver disease MASH.8

Trials behind the tablet

The label's blood sugar results come from 26-week trials against placebo and against three other diabetes drugs.1 Rybelsus 14 mg lowered HbA1c by 1.3 points against 0.9 for empagliflozin, and by 1.3 points against 0.8 for sitagliptin.1 Against daily liraglutide injection, it lowered HbA1c by 1.2 points against 1.1, with more weight loss (4.4 kg against 3.1 kg).1 In 324 adults with moderate kidney impairment, mean age 70, 14 mg lowered HbA1c by 1.0 point against 0.2 on placebo.1 Added to insulin in 731 adults, 7 or 14 mg lowered HbA1c by 0.9 to 1.3 points against 0.1.1

Safety points tied to the tablet

Rybelsus carries a boxed warning for thyroid C-cell tumors, which semaglutide caused in mice and rats; whether it does so in people is unknown.1 Breastfeeding advice comes from the tablet itself. In a lactation study, semaglutide in breast milk was too low to measure, but SNAC or its breakdown products are present in human milk and could build up in the infant. Because semaglutide products without SNAC exist, the label says breastfeeding is not recommended while taking Rybelsus or Ozempic tablets.1 The label also notes that slower stomach emptying can change how other pills are absorbed: levothyroxine exposure rose 33% when taken with semaglutide tablets.1

Storage is simple. Rybelsus is kept at room temperature, 68 to 77°F, in its original closed bottle to protect it from moisture, and not moved to any other container.1

The Rybelsus profile has the full label, and the semaglutide profile covers the molecule and its research. Frequently asked questions

Can you get semaglutide in a tablet?

Yes. As of October 2026, three FDA-approved products are semaglutide tablets taken once a day.29 Rybelsus (3, 7 and 14 mg) and Ozempic tablets (1.5, 4 and 9 mg) are for adults with type 2 diabetes.1 Wegovy tablets (1.5, 4, 9 and 25 mg) are for weight reduction and heart risk in adults.98 All three are prescription medicines, and each has its own starting dose and steps.29113

Do I need a prescription for semaglutide pills?

Yes, in the US. FDA lists every strength of Rybelsus, Ozempic tablets and Wegovy tablets with the marketing status Prescription.29 Each tablet starts at a low dose that is raised in steps, and the prescriber decides the dose and when to change it.113 Rybelsus and Ozempic tablets are not interchangeable milligram for milligram, so a switch between them follows the label's switching table rather than matching numbers.1

What is Rybelsus 3 mg used for?

Only for starting treatment. The label gives 3 mg once daily for days 1 to 30 as the starting dose, and says this dose is not effective for blood sugar control.1 On day 31 the dose rises to 7 mg, and from day 61 it can rise to 14 mg if more control is needed.1 The slow step-up is meant to reduce stomach and gut side effects, and the prescriber sets each change.1

Is Rybelsus the same as semaglutide?

Rybelsus is semaglutide in one particular form. Semaglutide is the active ingredient, also used in the Ozempic and Wegovy injections and tablets.781 Rybelsus is a 3, 7 or 14 mg tablet that adds the absorption enhancer SNAC and is taken daily for type 2 diabetes.1 Its milligram doses do not carry over to other semaglutide products: an injection is absorbed far more completely than a tablet, and even Ozempic tablets are not swapped with Rybelsus milligram for milligram.15
