Benefits and uses

Wegovy benefits and uses

In phase 3 trials in its label, Wegovy lowered weight, heart attack and stroke risk, and MASH liver inflammation.1 Adults without diabetes lost 14.9% of body weight on average over 68 weeks on 2.4 mg, against 2.4% on placebo.1 Among 17,604 adults with heart disease, heart attack, stroke or cardiovascular death occurred in 6.5% on Wegovy against 8% on placebo.1

Approved uses

FDA approves Wegovy for three uses, each alongside a reduced-calorie diet and more physical activity.1

  • Heart risk. Lowering the risk of cardiovascular death, heart attack and stroke in adults with established cardiovascular disease and either obesity or overweight.1
  • Weight. Reducing excess body weight and keeping it off long term in adults with obesity, or overweight plus at least one weight-related condition.1 The injection is also approved for this in children 12 and older with obesity.1
  • Liver. Treating noncirrhotic MASH with moderate to advanced fibrosis (F2 to F3) in adults, injection only.1 This is an accelerated approval, and continued approval may depend on a confirmatory trial.1

Wegovy benefits for weight

In the label's phase 3 trials, average weight loss ranged from 13.6% to 18.8% across the three options.1

  • 2.4 mg injection (STEP 1). 1,961 adults without diabetes, 68 weeks: -14.9% against -2.4% on placebo.1
  • 7.2 mg injection (Study 8). 1,407 adults with obesity, 72 weeks: -18.8%, against -15.5% on 2.4 mg and -3.9% on placebo.1
  • 25 mg tablets (OASIS 4). 307 adults without diabetes, 64 weeks: -13.6% against -2.4%.1

The label states that semaglutide lowers body weight with more fat mass loss than lean mass loss, which means some lean mass is lost too.5 Category results, weight regain after stopping and head-to-head trials are on Wegovy and weight loss.

Heart and blood vessels

SELECT (label Study 1) enrolled 17,604 adults aged 45 or older with established heart disease and a BMI of 27 or more, without diabetes.1 Over a median follow-up of 41.8 months, heart attack, stroke or cardiovascular death occurred in 6.5% on Wegovy against 8% on placebo.1 The hazard ratio was 0.80 (95% CI 0.72 to 0.90): a 20% lower hazard, or 1.5 percentage points in absolute terms.1 Weight fell 9.4% against 0.9% by week 104.1

FDA approved this use on March 8, 2024, calling Wegovy the first weight loss medicine also approved to lower this risk.42 The label says the exact mechanism behind the heart benefit has not been established.6 In the published trial report, adverse events led 16.6% on semaglutide and 8.2% on placebo to stop treatment for good, over a mean of 39.8 months.7 Evidence level: one large placebo-controlled phase 3 trial.1

Liver (MASH)

ESSENCE (label Study 11) is a phase 3 trial in adults with biopsy-defined MASH and F2 to F3 fibrosis.18 Its week-72 analysis covered the first 800 of 1,197 patients.8 MASH resolved without worse fibrosis in 63% on Wegovy against 34% on placebo.1 Fibrosis improved without worse MASH in 37% against 22%, and weight fell 10.5% against 2%.1 The label says semaglutide's mechanism in MASH is not fully understood and may involve weight loss and other pathways.6

Type 2 diabetes

Wegovy is not approved to treat type 2 diabetes, but its label includes trials in adults who have it.1 In STEP 2 (807 adults, 68 weeks), HbA1c fell 1.6 points against 0.4 from an average of 8.1%, and weight fell 9.6% against 3.4%.1 Details and the warnings that apply to this group are on Wegovy and blood sugar.

What is not established

  • Thyroid tumors in people. Semaglutide causes thyroid C-cell tumors in rodents; whether it does so in humans is unknown.1
  • Tablets in diabetes. The label says Wegovy tablets were not studied for weight reduction in adults with type 2 diabetes.1
  • Long-term liver outcomes. The MASH approval rests on week-72 biopsy results, and continued approval may depend on a confirmatory trial.1
  • Kidney protection. FLOW tested semaglutide 1 mg in 3,533 people with type 2 diabetes and chronic kidney disease.9 The risk of the main kidney outcome was 24% lower, but kidney risk reduction is an Ozempic use, not a Wegovy one.910
  • Brain health. Semaglutide is not approved for any cognitive use.11 EVOKE, a placebo-controlled trial of oral semaglutide in 1,840 people with early Alzheimer's disease, finished in January 2026 but had no posted results as of October 2026.11
  • After stopping. Weight came back in the label's withdrawal study when people switched to placebo.1 The figures are on Wegovy and weight loss.

Frequently asked questions

What was Wegovy made for?

It was made for weight: FDA first approved the injection on June 4, 2021, after a priority review.4 FDA's 2024 list of approved GLP-1 medicines shows Wegovy approved in 2021 for obesity or overweight.12 Its uses then widened: children 12 and older with obesity in December 2022, and a lower risk of heart attack, stroke and cardiovascular death in adults with heart disease and obesity or overweight in March 2024.42 MASH with liver fibrosis followed in August 2025 as an accelerated approval, and the daily tablets in December 2025.413

What are the benefits of Wegovy besides weight loss?

The label adds two uses beyond weight.1 In SELECT, adults with heart disease and overweight or obesity had a heart attack, stroke or cardiovascular death less often: 6.5% on Wegovy against 8% on placebo.1 That trial led to the 2024 heart risk approval.2 In ESSENCE, MASH resolved without worse liver scarring in 63% against 34% at 72 weeks, the basis of the 2025 accelerated liver approval.1 In adults with type 2 diabetes, HbA1c fell 1.6 points against 0.4, although Wegovy is not approved to treat diabetes.1

Do Wegovy pills work as well as the injection?

The label's results for the two forms come from separate trials.1 In OASIS 4, 307 adults on 25 mg tablets had an average weight change of -13.6% over 64 weeks, against -2.4% on placebo.1 In STEP 1, 1,961 adults on the 2.4 mg injection had -14.9% over 68 weeks, against -2.4%.1 The trials differed in size and length, so the figures are not a head-to-head comparison.1 The tablets are approved for weight and heart risk in adults, but not for MASH or for children.13

Is Wegovy approved for teenagers?

Yes, for the injection: a December 2022 supplement added children 12 and older with obesity.4 In STEP TEENS, 201 adolescents treated for 68 weeks saw BMI fall 16.1% on Wegovy against a 0.6% rise on placebo, and body weight change by -14.7% against +2.7%.1 Their maintenance dose is 2.4 mg or 1.7 mg weekly.1 The 7.2 mg dose is not established in children, and the tablets are approved for adults only.1
