Benefits and uses

Tirzepatide benefits and uses

In large trials of the approved injectable, tirzepatide lowered blood sugar in type 2 diabetes, cut average body weight by 15% to 21% over 72 weeks, reduced new diabetes cases in prediabetes and eased sleep apnea.12345 Each finding below names its evidence level. These results cannot be assumed to apply to research-labeled vials.6

What the evidence covers

Most findings come from randomized phase 3 trials of a once-weekly injection, some with thousands of adults.78 A few come from a phase 2 trial, from mice or from cell studies, which are earlier-stage evidence.91011 Several findings led to approved uses, while others, such as the liver results, still need larger and longer trials.12 SURMOUNT-1, the main weight trial, was supported by Eli Lilly, the maker.13

Approved uses

  • Blood sugar in type 2 diabetes (Mounjaro): approved with diet and exercise for adults and children 10 and older.14
  • Heart risk in type 2 diabetes (Mounjaro): approved in August 2026 to cut the chance of a major cardiovascular event (cardiovascular death, non-fatal heart attack or non-fatal stroke) in high-risk adults.1516
  • Weight (Zepbound): approved for long-term weight reduction and maintenance in adults with obesity, or with overweight plus a weight-related condition.17
  • Obstructive sleep apnea (Zepbound): approved in December 2024 for moderate to severe sleep apnea in adults with obesity.18 FDA called it the first drug treatment option for certain patients with the condition.19
  • European Union: Mounjaro is authorised for type 2 diabetes in adults and in children aged 10 and older, and for weight management in adults.2021

Reported benefits by evidence level

  • Blood sugar (phase 3, 478 and 1,879 adults): HbA1c fell about 1.9 to 2.3 percentage points over 40 weeks, more than with semaglutide 1 mg in the head-to-head trial.122 Detail: Tirzepatide and blood sugar.
  • Children and teens with type 2 diabetes (randomized trial, 99 patients aged 10 and older): both doses tested, 5 mg and 10 mg weekly, beat placebo on HbA1c over 30 weeks.2324
  • Weight in type 2 diabetes (phase 3, 478 adults): body weight dropped by 7.0 to 9.5 kg over 40 weeks.25 The trial recorded no clinically significant or severe hypoglycemia on tirzepatide.26
  • Insulin sensitivity (label, clamp study): 28 weeks of treatment made the body respond better to insulin in people with type 2 diabetes.27
  • Body weight (phase 3, 2,539 adults): weight dropped by an average of 15.0% to 20.9% over 72 weeks, against 3.1% with placebo.23 On 10 mg and 15 mg, half or more of participants (50% and 57%) lost at least a fifth of their weight.28 Detail: Tirzepatide and weight loss.
  • Diabetes prevention (phase 3, 1,032 adults with prediabetes): over 176 weeks, new type 2 diabetes was diagnosed in 1.3% of the tirzepatide groups and 13.3% of the placebo group.294
  • Sleep apnea (phase 3, two trials, 469 adults): breathing interruptions fell by 25.3 and 29.3 events per hour, versus 5.3 and 5.5 on placebo.30531
  • Waist size (phase 3b, 751 adults): waists shrank by 18.4 cm on tirzepatide and 13.0 cm on semaglutide over 72 weeks.3233
  • Heart and metabolic markers (phase 3, 2,539 adults): all prespecified cardiometabolic measures improved.34
  • Heart events in type 2 diabetes (phase 3, 13,299 adults): 12.2% of the tirzepatide group and 13.1% of the dulaglutide group had a major cardiovascular event.35 The result met the noninferiority goal but did not show superiority.35
  • Heart failure (randomized trial, 731 adults): in heart failure with preserved ejection fraction and obesity, cardiovascular death or worsening heart failure occurred in 9.9%, versus 15.3% on placebo.3637
  • Liver (phase 2, 190 adults): fatty liver disease with inflammation (MASH) resolved without worse scarring in 44% to 62%, versus 10% on placebo.38939 The authors called for larger, longer trials.12
  • Keeping weight off (phase 3, 670 adults): after 36 weeks on tirzepatide, continuing for another 52 weeks brought a further 5.5% loss, whereas switching to placebo led to a 14.0% weight gain.4041
  • A lower maintenance dose (phase 3b, 378 adults randomized): after 60 weeks at the full dose, stepping down to 5 mg kept modeled weight 16.6% below baseline at week 112, versus 9.9% on placebo.4243

Neither the heart failure result nor the liver result is among the approved uses in the current labels.14151744

What the evidence does not show

  • Muscle gain: not shown. According to the Zepbound label, the weight lost is more fat than lean mass, which means some lean mass goes too.45
  • Skin or hair benefits: none reported. Hair loss is a listed side effect, at 4% to 5% versus 1% on placebo.46
  • Other sleep problems: the sleep trials enrolled adults with obstructive sleep apnea.30 We found no trial of tirzepatide for insomnia.
  • A lasting effect after stopping: the SURMOUNT-4 authors found that stopping treatment led to substantial weight regain.47

Limits to keep in mind

  • Side effects: stomach and bowel effects were common, mostly during dose increases.34 See Tirzepatide side effects.
  • Study conditions: the results come from controlled trials in which weekly doses were raised on a set schedule, mostly over 40 to 72 weeks.487 Label schedules and the prescriber's role are on Tirzepatide dosage.
  • Research-labeled vials: FDA calls research-labeled GLP-1 products sold to consumers of unknown quality, so trial benefits say nothing reliable about such vials.6 Lab handling is covered in How to reconstitute Tirzepatide.

Frequently asked questions

Is semaglutide better than tirzepatide?

In the two head-to-head trials, tirzepatide produced larger average effects. In type 2 diabetes, HbA1c fell 2.01 to 2.30 points with tirzepatide versus 1.86 with semaglutide 1 mg over 40 weeks.22 In obesity, average weight loss over 72 weeks was 20.2% with tirzepatide and 13.7% with semaglutide 1.7 or 2.4 mg.4932 Stomach side effects were in a similar range in the diabetes trial: nausea 17% to 22% versus 18%, diarrhea 13% to 16% versus 12%, vomiting 6% to 10% versus 8%.50 Comparison: Ozempic vs Tirzepatide.

Does tirzepatide help sleep apnea?

In adults with obesity and moderate to severe obstructive sleep apnea, yes. In two 52-week trials with 469 adults, breathing interruptions fell by 25.3 and 29.3 events per hour on tirzepatide, versus 5.3 and 5.5 on placebo.30531 FDA approved Zepbound for this use on December 20, 2024.18 The approval covers adults with obesity only.44

Does tirzepatide lower the risk of heart attack or stroke?

In a trial of 13,299 adults with type 2 diabetes and established cardiovascular disease, major cardiovascular events occurred in 12.2% on tirzepatide and 13.1% on dulaglutide over a median of 210 weeks.3551 Tirzepatide was noninferior, but not shown to be superior.35 In August 2026, FDA added an indication for Mounjaro to lower this risk in high-risk adults with type 2 diabetes.1552

Does tirzepatide build muscle?

No trial reviewed here showed muscle gain. The Zepbound label says tirzepatide lowers body weight with greater fat mass loss than lean mass loss, which means some lean mass is also lost.45 In mice, the weight lost on tirzepatide came mainly from fat.10 Strength and muscle function were not among the results in the sources for this page.
