Pen use
Mounjaro injection sites and pen use
Mounjaro needs no reconstitution: every pen and vial holds a ready-to-inject solution.1 It is injected under the skin of the abdomen or thigh, or the back of the upper arm when another person gives it, once a week at a new spot each time.1 Unopened pens and vials are refrigerated at 36°F to 46°F.1

Nothing to mix
Every Mounjaro pen and vial holds a finished solution, and the Instructions for Use for the pen, KwikPen and vials include no mixing step.1 The liquid is clear and colorless to slightly yellow, with a pH of 6.5 to 7.5.1 Besides tirzepatide, it contains sodium chloride, sodium phosphate dibasic heptahydrate and water for injection; the multi-dose vial and KwikPen add benzyl alcohol, glycerin and phenol.1
The four presentations
| Presentation | Holds | Each injection needs |
|---|---|---|
| Single-dose pen, 4 per carton | One 0.5 mL dose1 | The four pen steps described below1 |
| Single-dose vial, 1 per carton | One 0.5 mL dose1 | A syringe that can measure 0.5 mL, with a new syringe and needle1 |
| Multi-dose vial | Four 0.6 mL doses in 2.4 mL1 | A syringe that can measure 0.6 mL, with a new syringe and needle every time1 |
| KwikPen | Four fixed 0.6 mL doses in 2.4 mL1 | A new pen needle (not included) and priming before each dose1 |
Choosing a Mounjaro injection site
The label lists three areas: the stomach area (abdomen), the thigh, and the back of the upper arm, which is used when another person gives the injection.1 The needle goes under the skin, never into a muscle or a vein.1 For the abdomen, the KwikPen instructions keep injections at least 2 inches from the belly button.1 On average, 80% of a dose given under the skin reaches the bloodstream, and exposure was similar from the abdomen, thigh and upper arm.2
For any prescribed injection under the skin, MedlinePlus lists the outer upper thighs, and the belly between the ribs and the hip bones, as self-injection areas, always on healthy skin free of redness, swelling, scarring or bruising.4 For the back of the arm, the NHS suggests asking someone to help.8
Rotating sites
The label calls for rotating the injection site with every weekly dose: the same body area may be used again, but at a new spot.1 MedlinePlus advises moving each injection at least 1 inch from the previous one, to keep the skin healthy and help absorption, and suggests keeping a chart of spots already used.45 Injection site reactions were reported in 3.2% of people on Mounjaro, against 0.4% on placebo.1
Using each presentation
Single-dose pen
The Instructions for Use set out four steps: choose the site, pull off the gray base cap, place the clear base flat on the skin and release the lock, then press and hold the purple button for up to 10 seconds.1 A first click means the injection has started and a second click that it is complete, when the gray plunger comes into view.1 The pen needs no warming first. It has glass parts, so one dropped on a hard surface is not used.1
KwikPen
Each KwikPen holds four fixed weekly doses. A new pen needle is attached and the pen is primed before every injection.1 The dose knob is turned until it stops, then pushed in and held while slowly counting to 5.1 The label does not recommend the KwikPen for self-injection by children or by people with visual impairment.1 It is never shared, even with a new needle, because of the risk of blood-borne infection.1
Vials
Both vial types need a syringe that measures 0.5 mL or 0.6 mL, such as a 1 mL syringe, and a new syringe and needle each time.1 General guides on drawing from a vial add two habits: wipe the rubber top with alcohol, and write the opening date on any vial used more than once.6
Dose volume depends on the presentation. A single-dose vial holds 2.5 mg to 15 mg in 0.5 mL (5 to 30 mg per mL), while a multi-dose vial holds 10 mg to 60 mg in 2.4 mL, split into four 0.6 mL doses (about 4.2 to 25 mg per mL).1 So a 2.5 mg dose is 0.5 mL from one vial and 0.6 mL from the other. That is arithmetic on label figures; the prescription and the product's instructions set what is drawn up. FDA has said confusion among milligrams, milliliters and syringe "units" may have contributed to dosing errors with compounded semaglutide vials, in which some patients injected 5 to 20 times the intended dose.7
Before each injection
A healthcare provider trains the patient or caregiver on the exact presentation before the first dose, and again if it changes.1 Each time, the liquid is checked. It should be clear and colorless to slightly yellow, and it is not used if it is cloudy, discolored, holds particles or has been frozen.1 General guides differ on the skin: one MedlinePlus guide wipes the site with an alcohol pad, while its insulin guide uses clean, dry skin and no alcohol wipe.45 The Mounjaro Instructions for Use start with hand washing and use an alcohol swab only on the KwikPen's inner seal or a vial's rubber stopper; they include no step for wiping the skin.1
With insulin
Mounjaro and insulin are given as separate injections and are never mixed.1 They may go into the same body region, but not right next to each other.1 Prescribers may also lower the insulin dose when Mounjaro starts; see Mounjaro dosage.1
Storage
- Before first use: every presentation is kept in the refrigerator at 36°F to 46°F (2°C to 8°C), in its carton to protect it from heat and light.1
- Never frozen: Mounjaro that has been frozen is not used.1
- Out of the refrigerator: up to 86°F (30°C) for a total of 21 days for single-dose pens and vials, and 30 days for multi-dose vials and KwikPens.1
- KwikPen: stored with the cap on and no needle attached.1
- Children: all Mounjaro is kept out of their reach.1
Disposal
FDA counts needles, syringes and auto-injector pens as sharps, and it advises putting them in a sharps disposal container right after use.910 Cleared containers are rigid plastic with a fill line.11 A heavy-duty household bottle, such as a laundry detergent jug, can stand in if it is leak-resistant, stays upright, has a tight, puncture-resistant lid and is labeled as hazardous waste.911 At about three-quarters full, local rules apply: drop-off sites, household hazardous waste sites, mail-back programs or pick-up.10 Loose needles never go in household trash, recycling bins or the toilet, and containers are not reused.91210 The Mounjaro Instructions for Use put a used single-dose pen, and every needle and syringe, straight into a sharps container; a KwikPen with its needle removed may go in household trash or a sharps container.1

Frequently asked questions
Where do you inject Mounjaro?
Into the fatty tissue just under the skin, since Mounjaro is a subcutaneous injection.14 The label names the stomach area (abdomen) and the thigh, plus the back of the upper arm when another person gives the injection, and never a muscle or a vein.1 KwikPen users keep injections in the abdomen at least 2 inches from the belly button.1 Drug exposure was similar from all three areas, and the exact spot changes every week.21
Where in the thigh is Mounjaro injected?
The Mounjaro label lists the thigh as one of its injection areas.1 A MedlinePlus patient guide points to the outer side of the upper thigh for self-injection, and the NHS describes the fatty areas of the thigh for weekly tirzepatide.48 The skin should be healthy, with no redness, swelling, scarring or bruising, and each injection goes at least 1 inch from the last one.4
Does Mounjaro need to be mixed or reconstituted?
No. All four presentations (single-dose pen, single-dose vial, multi-dose vial and KwikPen) hold a finished liquid, and none of their Instructions for Use includes a mixing step.1 Tirzepatide that arrives as a powder needing mixing is not Mounjaro.1 FDA says unapproved versions do not undergo its review for safety, effectiveness and quality before they are marketed.3
How long can Mounjaro stay out of the fridge?
It depends on the presentation. A single-dose pen or vial may be kept unrefrigerated at up to 86°F (30°C) for a total of 21 days, then it is thrown away.1 An unopened multi-dose vial or KwikPen kept at room temperature is discarded after 30 days.1 Once a multi-dose vial or KwikPen is in use, it can stay in the refrigerator or at room temperature up to 86°F, and it is discarded 30 days after first use or after 4 doses, even if medicine is left.1
