Appointment requests & messages
Any appointment request or message you send through this site is a request, not a confirmed booking. An appointment is confirmed only when the practice confirms it with you directly.
Please do not send urgent or emergency information through this website. If you are having an eye emergency, call the office at 239-500-2020 during opening hours, or seek in-person care right away. In a life-threatening emergency, call 911.
Please also avoid sending sensitive medical detail through a web form. If we need that information, we will collect it properly, in person or by a secure route.
